2.1. The Subject Of The Order Is The Implementation Of The Sentence Entitled: Purchase Of Equipment For Therapeutic Rehabilitation For Oncological Patients Implemented Under Agreement No. 1/18/24/2025/305/316 For The Granting Of A Targeted Subsidy Under The Multi-Annual Program Entitled: National Oncological Strategy For Sp Zoz Csk Um In Łódź.2.2. Due To The Extensive Description - A Detailed Description Of The Subject Of The Order, Details Of The Implementation Of The Subject Of The Order, Etc., Are Included In Annex No. 8 To The Swz - Description Of The Subject Of The Order - Technical Parameters (Hereinafter Referred To As Opz). 2.3. The Contractor Is Obliged To Attach To The Offer, In Particular, Appendix No. 8 To Swz - Opz - Technical Parameters, Completed In Accordance With The Requirements Of The Ordering Party. 2.4. Failure To Submit Annex No. 8 To The Swz Constitutes The Inability To Evaluate The Offer And Thus The Basis For Rejecting The Offer From The Procedure. 2.5. The Subject Of The Order Has Been Divided Into 4 Parts, The Contractor May Submit An Offer For Any Number Of Parts. Package 1: Kinesitherapy Device - 4 Pcs. Cpv Code: 33100000 Medical Devices, 37440000-4 - Physical Exercise Equipment, 33155000 - Physical Therapy Equipment, 33190000-8 - Miscellaneous Medical Devices And Products, 2.7. The Ordering Party Reserves, In Accordance With Art. 310 Of The Public Procurement Act, That It May Invalidate The Contract Award Procedure If The Public Funds That The Contracting Authority Intended To Use To Finance All Or Part Of The Contract Were Not Granted To It, And The Possibility Of Invalidating The Procedure On This Basis Was Provided For In The Contract Notice - In The Procedure Conducted Under The Basic Procedure. 2.8. The Delivered Subject Of The Order Must Have Operating Instructions In Polish (And How Many Are Required), Have The Appropriate Documents Referred To In The Swz, And Must Be Covered By At Least The Warranty Period - Specified For A Given Device Or Equipment, Specified In Annex No. 8 To The Swz. If There Are Any Doubts As To Whether The Offered Equipment Or Devices Meet The Technical Parameters, The Ordering Party Is Entitled To Request Documents Confirming The Offered Technical Parameters Of The Equipment Or Devices, E.G. Catalog Cards, Certificates, Etc. 2.9. The Equipment Delivered To The Ordering Party Must Meet The Following Conditions: A) The Delivered Subject Of The Order Must Be Brand New, Not For Exhibition And Free From Third Party Rights, B) Have A Warranty Period Not Shorter Than That Specified For Individual Devices And Equipment In Annex No. 8 To The Swz, And Unless Specified In The Opz, It Is 24 Months. C) The Devices Offered On The Day Of Submission Of Offers Cannot Be Intended By The M
