The Subject Of This Order Is The Delivery, Installation, Commissioning, Staff Training And Commissioning Of The 1.5T Magnetic Resonance System And The X-Ray Machine Along With The Adaptation Of Rooms Based On Design Documentation For The Needs Of The Specialist Hospital. A. Falkiewicz In Wrocław And Relocation Of The Densitometer From Another Hospital Building. 2. The Scope Of The Order Includes: 1) Carrying Out Construction Works According To The Design Documentation Developed And Approved By The Ordering Party And Granting At Least 60-Month Warranty On The Terms Specified In § 5 Of The Draft Provisions Of The Contract, Constituting No. 7 Of The Swz (In Accordance With The Contractors Declaration Submitted In The Offer); 2) Delivery, Installation And Commissioning Of Medical Devices Listed In Annex No. 3 To The Swz; A) Delivery, Assembly / Installation (In The Place Indicated By The Ordering Party, Ensuring Collision-Free And Ergonomic Use), Commissioning, Performance Tests And Commissioning Of The Equipment Constituting The Subject Of The Order; B) Conducting Acceptance Tests In Accordance With The Regulations, Enabling Obtaining Permission To Commission And Use X-Ray Machines In A Medical X-Ray Laboratory; C) Conducting Training In The Operation And Basic Maintenance Of The Supplied Equipment For The Ordering Partys Staff, Within The Deadline Agreed With The Ordering Party (In Accordance With The Opz); D) Granting At Least 24-Month Warranty On The Terms Specified In § 5 Of The Draft Provisions Of The Contract, Constituting No. 7 Of The Swz (In Accordance With The Contractors Declaration Submitted In The Offer). 3. Relocation Service Of The Device Owned By The Ordering Party: Disassembly, Securing All Elements Of The Device For The Time Of Relocation, Relocation Between Facilities Using The Contractors Resources (Human, Technical And Hardware Resources Necessary To Perform The Service Will Be Provided By The Contractor), Assembly At The Target Location, Calibration And Commissioning And Commissioning, As Well As Performance Of A Technical Inspection. 4. A Detailed Description Of The Subject Of The Order Is Included In The Documentation Attached To The Swz: 1) Description Of The Subject Of The Order (Opz) In The Scope Of Supplies And Services - Annex No. 3 To The Swz, 2) Construction Design - Annex No. 8 To The Swz, 3) Detailed Design In The Industries Covered By The Order - Annex No. 9, 4) Technical Specification For The Execution And Acceptance Of Construction Works In The Industries Covered By The Order - Annex No. 10, 5) Bills Of Quantities Of Works Covered By The Order - Annex No. 11. 6) Draft Provisions Of The Contract Constituting - Annex No. 7 To The Swz.
