1. The Subject Of The Order Is The Provision Of Specialized Care Services For People With Mental Disorders From The Area Of The Sędziszów Małopolski Commune For 2026. 2. The Provision Of Specialized Care Services For People With Mental Disorders For Clients Of The Municipal And Communal Social Welfare Center In Sędziszów Małopolski Will Take Place At The Place Of Residence Of These People On Working Days, Excluding Statutory Holidays And Holidays, From 7:00 A.M. To 1900.3. Specialized Care Services Are Understood As Services Specified In The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Consolidated Text: Journal Of Laws Of 2024, Item 816), Hereinafter Referred To As The Suo Regulation, I.E. Care Services Provided By People With Specialized Professional Training, Tailored To The Specific Needs Of People Requiring Assistance In This Form, Resulting From The Type Of Their Disease, - Including Specialist Care Services For People With Mental Disorders.4. These Services Should Be Adapted To The Specific Needs Resulting From The Type Of Disease.5. The Estimated Number Of Clients For Whom Specialist Care Services Will Be Provided In 2026 Will Be Approximately 15 People.6. The Estimated Number Of Hours Of Specialist Care Services To Be Provided By The Contractor During The Duration Of The Contract Will Be 3,843 Hours.7. The Ordering Party Stipulates That The Number Of Hours Multiplied By The Amount Resulting From The Selected Offer May Not Exceed Pln 1,200 Per Month Per Person.8. The Cost Of One Hour Of Remuneration For The Provision Of Specialist Care Services May Not Exceed Pln 56.20 Gross.9. The Parties Allow For The Possibility Of Reducing Or Increasing The Number Of People And Hours In The Scope Of Services Provided, According To The Needs Of The Ordering Party, And Their Actual Size Will Result From The Hours Actually Provided. In The Event Of A Reduction Or Increase In The Number Of Hours, Payment Will Be Made For The Hours Actually Completed. In Such A Case, The Unit Prices Offered By The Contractor Will Remain Unchanged.10. Information On The Provision Of Services, Their Scope, Date And Amount Will Be Provided By A Person Designated By The Ordering Party On The Basis Of An Administrative Decision Issued By The Ordering Party.11. The Ordering Party Reserves The Right To: A) Substantive Control Of The Services At The Place Of Their Provision; B) Correction Of Service Hours For Persons Qualified To Use The Services; C) Individual Conversation With The Contractors Employees Providing Services Covered By This Agreement; D) Submitting A Request To The Contractor To Change The Person Providing The Services, If There Are Circumstances Justifying The Change.12. As Part Of The Performance Of The Subject Of The Order, The Contractor Is Obliged To Provide An Appropriate Number Of People With Qualifications To Provide Care Services For People With Mental Disorders And Work Experience In Accordance With The Su
