Description Of The Subject Of The Order1. The Subject Of The Order Is The Supply Of Medical Devices, I.E. Respirators For The Independent Public Health Care Center Of The District Emergency Service In Sosnowiec For The Needs Of The Ordering Party In Accordance With The Assortment Described In Detail In Annexes No. 1A-1B To This Swz, Which Also Constitutes The Detailed Description Of The Subject Of The Order For Tasks No. 1 To No. 6, As Divided Below: 1) Task No. 1 - Respirator 7 Pieces2) Task No. 2 - Respirator For A Motor Ambulance 1 Piece2. The Ordering Party Envisages Submitting Offers For The Entire Order Or For Parts, Where A Given Task Constitutes Part Of The Order According To The Division As Above. The Ordering Party Does Not Limit The Number Of Parts Of The Contract That May Be Awarded To One Contractor.3. In Each Item Of The Assortment Forms (Appendices No. 1A To No. 1B According To The Division For Individual Tasks), The Contractor Will Provide The Manufacturer/Manufacturer, Trade Name Of The Product And/Or Catalog Number, If Assigned - In Accordance With The Requirements Specified In The Price Forms.4. In Annexes No. 1A-1B To This Tor, Which Is Also The Detailed Description Of The Subject Of The Order For Tasks No. 1 To No. 2, The Minimum Required Technical And Operational Parameters Of The Subject Of The Order Are Described.5. The Ordering Party Requires That The Subject Of The Order Be Brand New, In Complete Condition, Without The Need To Make Additional Deliveries Or Incur Costs, And Cannot Be The Subject Of Rights Or Claims Of Third Parties.6. The Subject Of The Order Should Come From Current Production, Be Free From Physical And Legal Defects, Of The Highest Quality And Admitted To Trading In The Eu. Moreover, No Element Of The Subject Of The Order Or Any Of Its Components May Be Reconditioned, Used For Exhibition Purposes Or Previously Used By Another Entity For Any Purpose.7. The Offered Products Within The Scope Of The Subject Of The Order Must Have A Certificate Or Declaration Of Conformity Or Authorization Allowing Marketing And Use In Medical Entities, As Well As Appropriate Labeling In Accordance With The Law. 8. All Products Indicated In The Offer Must Be Admitted To Trading In The Eu. 9. The Ordering Party Requires The Delivery Of Documentation Of The Subject Of The Order In Polish, Including User Manuals, Quality Guarantees, Technical Documentation, Certificates Or Declarations Of Conformity, Passports And Other Documentation In Accordance With Legal Regulations - Together With The Delivery Of The Device. 10. The Contractor Is Responsible For The Quality Of The Entire Subject Of The Order And The Quality Of The Materials Used. 11. Delivery Of The Subject Of The Order Takes Place At The Contractors Own Expense And Risk. 12.
