1. The Subject Of The Order Is The Provision Of Occupational Medicine Services For Employees Of The Nursery Complex Of The Capital City Of Warsaw. Warsaw.2. The Subject Of The Order Includes: 1) Carrying Out Preventive Examinations Of The Ordering Partys Employees: Preliminary, Periodic And Control Examinations As Well As Sanitary And Epidemiological Examinations Provided For In: A) The Act Of June 26, 1974, Labor Code, Hereinafter Referred To As The Labor Code, B) The Act Of December 5, 2008 On Preventing And Combating Infections And Infectious Diseases In Humans (Hereinafter Referred To As The Act), C) The Regulation Of The Minister Of Health And Social Welfare Of May 30 1996 On Medical Examinations Of Employees, The Scope Of Preventive Health Care For Employees And Medical Certificates Issued For The Purposes Provided For In The Labor Code, In Accordance With The Order Issued By The Ordering Party (Hereinafter Referred To As The Regulation),2) Performance Of Laboratory Tests Ordered By An Occupational Medicine Doctor,3) Issuing Medical Certificates For The Purposes Specified In The Labor Code And Implementing Regulations, In Accordance With The Applicable Templates, In Two Copies,4) Participation Of The Doctor Providing Health Care In Work Of The Occupational Health And Safety Committee At The Request Of The Ordering Party, Appointed In Accordance With The Procedure Specified In Art. 237(12) Of The Labor Code,5) Maintaining And Storing Medical Documentation In The Field Of Preventive Health Care In Accordance With Applicable Regulations,6) Preparing And Storing Medical Documentation In Accordance With The Regulation Of The Minister Of Health Of July 29, 2010 On The Types Of Medical Documentation Of The Occupational Medicine Service, The Method Of Its Management And Storage And Templates Of Documents Used.3. The Ordering Party Determines The Employment Requirements Based On The Employment Relationship In The Circumstances Referred To In Art. 95 Of The Public Procurement Law:1) The Ordering Party Requires Employment Under An Employment Contract Of At Least ½ Full-Time By The Contractor Or Subcontractor Of Persons Performing The Activities Indicated Below During The Execution Of The Order: Administrative Activities (Registration Of Patients).2) The Ordering Party Requires That The Employment During The Execution Of The Order Lasts Throughout The Entire Period Of Execution Of The Order, In The Event Of Termination Of The Employment Relationship By The Employed Person Or By The Contractor / Subcontractor Before The End Of This Period, The Contractor / Subcontractor Will Be Obliged To Employ Him In This Period.
