1. The Subject Of The Order Is The Provision Of Veterinary Services For Newly Caught Animals And Animals Staying In The Place Of Temporary Keeping Of Caught Animals, Located At Ul. Krzywa 18 In Zielonka. 1) Veterinary Services For Animals At The Place Of Temporary Detention Of Caught Animals Will Consist In Arriving At The Request Of The Ordering Party To The Above-Mentioned. Places For Collective Vaccinations, Deworming And In Case Of Emergencies Requiring Medical Intervention. In Addition, Animals Caught By The Ordering Party Will Be Brought To The Contractors Clinic. After A 24-Hour Stay At The Clinic, Which Includes: Clinical Examination, Provision Of Emergency Assistance, Deworming, Flea Removal, Tick Removal, Checking Whether The Animal Is Microchipped (If Not, It Should Be Chipped) And After Obtaining A Veterinary Inspection Book, They Will Be Delivered By The Contractor To The Place Of Temporary Keeping Of The Caught Animals Located At Ul. Krzywa 18 In Zielonka. In The Case Of Cats, After Consultation With The Ordering Party, A Test For Fiv/Felv Must Be Performed. In Justified Cases (I.E. The Animals Health Condition Is Serious And Very Serious, Moribund, Aggressive Animal Posing A Threat To Humans, Terminally Ill Animal), Euthanasia Should Be Performed After Consultation With A Representative Of The Environmental Protection Department Of The City Hall In Zielonka - Each Case Must Be Documented With A Note Prepared By A Veterinarian, Which Should Be Attached To The Monthly Reports Of Veterinary Services Performed. Other Necessary Medical Treatments, Apart From Those Listed In The Table, Will Be Performed According To The Order And Scope Agreed Each Time With A Representative Of The Environmental Protection Department Of The Zielonka City Hall. 2. Pursuant To The Content Of Art. 95 Section 1 Of The Public Procurement Law, The Ordering Party Requires The Contractor Or Subcontractor To Employ, Under An Employment Contract Within The Meaning Of The Provisions Of The Act Of June 26, 1974 - Labor Code (Consolidated Text: Journal Of Laws Of 2025, Item 277, As Amended), Persons Performing Activities Related To The Implementation Of The Subject Of The Order, Such As: Veterinarians, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Article 22 § 1 Of The Act Of June 26, 1974 - Labor Code (Journal Of Laws Of 2025, Item 277, As Amended). 3. Detailed Requirements For The Employment Of Employees And The Method Of Verification And Control Of The Contractors Or Subcontractors Compliance With The Requirements Related To The Employment Of These Persons Are Specified In § 11 Of The Draft Contract, Constituting Annex No. 3.1 To The Swz. 4. The Detailed Scope Of The Order Is Presented In Annex No. 4.1 To The Swz.
