The Subject Of The Order Are Medical Services In The Field Of Occupational Medicine For People Employed Or Applying For Employment At The District Educational Finance Office - Ursynów, Capital City. Warsaw Or In Selected Educational Institutions In The Ursynów District Of The Capital City Of Warsaw. Warsaw.2. Medical Services In The Field Of Occupational Medicine Are Those That The Ordering Party Is Obliged To Provide Under The Labor Code And The Act On Occupational Medicine Services.3. The Subject Of The Order Includes: 1) 425 Initial, Periodic Or Control Examinations; 2) 185 Ophthalmological Examinations; 3) 89 Laryngological Examinations; 4) 179 Issuance Of Medical Certificates For Sanitary And Epidemiological Purposes; 5) 34 Resting Ecgs; 6) 15 Lipid Profiles; 7) 11 Tests For The Purpose Of Ruling On The Need To Grant A Teacher Leave For Health Reasons Together With Issuing The Certificate;8) 9 Glucose Tests.4. The Ordering Party Requires That A Set Of Tests To Which An Employee Or A Person Applying For Employment Is Directed To Be Performed From Monday To Friday, Except For Days Off From Work, Between: From 8:00 A.M. To 4:00 P.M. During A Visit To One Medical Facility And Within No More Than 3 Business Days Of Reporting The Person For Examination.5. Working Days Are Considered To Be Days From Monday To Friday, Except Public Holidays.6. Additional Specialist Consultation Examinations Referred To In The Regulation On Medical Examinations Of Employees Are Carried Out On The Basis Of A Doctors Referral And At The Expense Of The Contractor At A Facility Indicated By Him In The Capital City. The Ordering Party Requires That The Order Be Performed On The Terms And Conditions Described In The Draft Provisions Of The Agreement, Which Constitute Annex No. 9 To Swz.8. In This Order, The Ordering Party Applies The Social Clause Pursuant To Art. 95 Section 1 Of The Act Pzp.9. The Contractor Undertakes To Employ, Under An Employment Contract By The Contractor Or Subcontractor, Persons To Perform The Following Activities During The Execution Of The Order: 1) Setting The Date Of Tests In Accordance With The Principles Described In § 4 Section 7 Of The Draft Provisions Of The Contract, Which Constitute Annex No. 9 To The Swz; 2) Supervising The Proper Implementation Of This Order; - Except For The Situation In Which The Above-Mentioned Activities Are Performed By The Contractor Who Is A Natural Person Running A Business.
