Tenders Are Invited For Providing Specialized Care Services For People With Mental Disorders From The Radomyśl Wielki Commune

1. The Subject Of The Order Is: Provision Of Specialized Care Services For People With Mental Disorders, Including Autism, From The Radomyśl Wielki Commune. The Subject Of The Order Applies To Both Adults And Children. 5. Specialized Care Services Will Be Provided For 22 People With Mental Disorders. The Number Of Hours Of Specialist Care Services For The Entire Order Will Be: 2,332 Hours, Subject To Section 6 Point 4 Of The Swz. 6. Detailed Description Of The Subject Of The Order: 1) The Provision Of Specialist Care Services Will Be Based On The Provisions Of: A) The Act On Social Assistance Of March 12, 2004 (Consolidated Text: Journal Of Laws Of 2025, Item 1214); B) Act On Mental Health Protection Of August 19, 1994 (Consolidated Text: Journal Of Laws Of 2024, Item 917); C) Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Consolidated Text: Journal Of Laws Of 2024, Item 816), Hereinafter Referred To As The Regulation. 2) The Task Of The Contractor Of The Subject Order Is To Provide Specialized Care Services For People With Mental Disorders. 3) Specialized Care Services For People With Mental Disorders Will Be Provided To Beneficiaries Of The Municipal Social Welfare Center In Radomyśl Wielki Entitled To Use These Services On The Basis Of An Administrative Decision. The Decision Specifies The Place, Time And Monthly Number Of Hours Of Specialized Services Granted. The Task Of The Contractor Of The Subject Order Is To Provide Specialized Care Services For No More Than 7 Hours. Per Week After Consultation With The Person And, In The Case Of Children, With The Parent/Legal Guardian, From Monday To Friday From 700-2000 For People Residing In The Radomyśl Wielki Commune, Indicated By The Ordering Party In The Period Until December 31, 2026.4) The Parties Allow For The Possibility Of Reducing Or Increasing The Number Of People And Hours In The Scope Of Services Provided, To Meet The Needs Of The Ordering Party, And Their Actual Amount Will Result From The Hours Actually Performed. In The Event Of A Reduction Or Increase In The Number Of Hours, Payment Will Be Made For The Hours Actually Completed. In Such A Case, The Unit Prices Offered By The Contractor Will Remain Unchanged. The Ordering Party Reserves The Right To Increase The Estimated Number Of Hours Of The Service Being The Subject Of The Order To A Maximum Of 30%, As Needed, Taking Into Account The Gross Price Offered By The Selected Contractor For One Hour Of Service Provision. If The Ordering Party Does Not Exercise The Option Right, The Contractor Is Not Entitled To Any Claims, Including Compensation For Failure To Exercise The Option Or Its Part, And It Cannot Be The Basis For Refusing To Perform The Order. The Ordering Party Reserves The Right To Reduce The Estimated Number Of Hours Of The Service That Is The Subject Of The Order, According To Current Needs, And The Ordering Party Guarantees The Implementation Of At Least 50% Of The Estimated
