Providing Services In The Scope Of Copyright Supervision Of The Amms/Infomedica Plus Application Software At The Specialist Hospital In Zabrze Sp. Z O. O., Case No. Dzp/24 Tp/2025. A Detailed Description Of The Subject Of The Order (List Of Modules Covered By Supervision) Is Attached As Annex No. 3 To The Swz - Assortment And Price Form. Conditions Required From Contractors - Contractors Submitting An Offer In The Procedure Declare That They Undertake To Comply With The Following Requirements: 1) They Will Offer A Service Consistent With The Description Of The Subject Of The Order Specified In Annex No. 3 To The Swz - Form Assortment And Price.2) They Will Present A Certificate/Document From The Manufacturer Of The Amms/Infomedica Software, Asseco Poland S.A. Confirming The Contractors Rights To Distribute Amms/Infomedica Software And Provide It Services Related To It.3) They Will Offer A Payment Deadline Of 30 Days From The Date Of Receipt By The Ordering Party Of A Correctly Issued Vat Invoice.4) They Will Sign An Agreement, The Template Of Which Is Annex No. 4 To The Swz.5) The Ordering Party, In Accordance With Art. 95 Of The Public Procurement Law Requires That Activities Related To The Performance Of The Duties Of The Project Manager As Part Of The Implementation Of The Contract Be Performed By Persons Employed By The Contractor Or Subcontractor Under An Employment Contract In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code (Consolidated Text: Journal Of Laws Of 2025, Item 277).6) The Ordering Party Requires That, Throughout The Period Of Contract Implementation, The Contractor Must Have A Valid Civil Liability Insurance Contract In The Scope Of Activities Related To The Subject Of The Order, For An Amount Not Less Than The Value Of The Offer For Which The Contractor Concluded A Contract With The Ordering Party. The Contractor Will Provide The Ordering Party With A Copy Of The Document Confirming Civil Liability Insurance (Together With Proof Of Payment Of The Premium Or An Installment Of The Policy Premium), Before Signing The Contract, Which Will Constitute An Annex To The Contract.7) Due To The Implementation Of The Internal Reporting Regulations Regulating The Whistleblower Protection Procedure At The Ordering Party, The Contractor Undertakes To Make All Its Employees, Subcontractors And Other Persons Involved In The Implementation Of The Order Familiar With Its Content. The Internal Reporting Regulations Regulating The Whistleblower Protection Procedure Are Available At The Secretariat Of The Ordering Partys Management Board.
