The Subject Of The Order Is The Provision Of Rehabilitation Services For 16 Participants Of The Senior+ Day Care Home In Ołpiny In The Form Of Individual And Group Classes.1) Participants Of The Senior+ Day Care Home In Ołpiny Are People Who Need Support In Everyday Functioning And Who, Due To Their Health Condition Or Disability, Require Care Or Support Due To The Inability To Perform At Least One Of The Basic Everyday Activities.2) The Provision Of The Service Is To Consist In Particular On: A) Conducting Individual Physical Rehabilitation With Dds+ Participants, In Accordance With Doctors Recommendations, B) Conducting Physical And Sports Activities: - Active Exercises, - Passive Exercises, - Supportive Exercises, - Resistance Exercises, - Weight-Bearing Exercises, - Breathing Exercises, - Exercises With Devices Available In Dds+, C) Conducting Massages According To The Individual Needs Of Dds+ Patients, D) Keeping Documentation, E) Cooperation With The Care And Therapeutic Team And Project, F) Diagnosing Dysfunctions Of Patients Motor Organs, G) Explaining To Participants The Need And Purposefulness Of The Exercises Performed, H) Periodic Analysis Of Activities Resulting From Individual Action Plans. 3) Basic Information: A) Number Of Participants: 16 B) Number Of Hours: 360 C) Task Implementation Period: Up To 12 Months From The Date Of Awarding The Order. 4) Hours Of Providing Services: Monday To Friday On Working Days From 8:00 A.M. To 4:00 P.M. The Subject Of The Order, Taking Into Account The Individual Needs And Capabilities Of The Participants. B) Preparation Of Materials, I.E. Class Program And Auxiliary Materials - The Contractor Is Obliged To Mark The Prepared Materials With Logos Indicated By The Ordering Party. The Ordering Party Will Make Them Available To The Contractor After Concluding The Contract, C) Keeping Documentation Of Classes Conducted, I.E. Attendance Lists, Class Logs, Work Time Sheets, D) Storing Documentation Of Classes Conducted In A Way That Ensures Availability, Confidentiality And Security And Systematically Transmitting It, E) Cooperation With The Dds+ Manager And Other Staff, F) Reporting Identified Irregularities, G) Making Documentation Related To The Subject Of The Order Available To Entities Authorized To Inspect The Implementation Of The Project.
