The Subject Of The Order Is The Provision Of Personal Assistance Services Referred To In The Personal Assistant For Persons With Disabilities Program For Local Government Units - 2026 Edition, Hereinafter Referred To As The Service To The Residents Of The Międzyrzecz Commune Who Are Participants In The Program. Services Provided In The City Of Międzyrzecz Commune, I.E. In The Place Of Residence Of The Resident Covered By The Services And In His Immediate Surroundings. 1.2. The Personal Assistance Service Will Be Provided To Persons Qualified For The Program Who Meet The Criteria Specified In The Program1) Personal Assistant For Persons With Disabilities Program For Local Government Units - Edition 20262) Https://Www.Gov.Pl/Web/Uw-Opolski/Asystent-Osobisty-Osoby-Z-Niepelnosprawnoscia-Dla- Jednostek-Samorzadu-Terytorialnego---Edycja-20261.3. In The Case Of Aoon Program Participants, Services Can Be Provided 24 Hours A Day, 7 Days A Week, By The Same Assistant For A Maximum Of 12 Hours A Day. 1.4. The Main Goal Of The Personal Assistant For A Person With A Disability Program For Local Government Units - 2026 Edition Is To Support Disabled People, Including: 1) Children From The Age Of 2 To The Age Of 16 With A Disability Certificate, Including The Indications In Point 7 And 8 In The Decision: The Need For Permanent Or Long-Term Care Or Assistance Of Another Person Due To A Significantly Limited Ability To Live Independently And The Need For Constant Daily Participation Of The Childs Guardian In The Process Of Treatment, Rehabilitation And Education Of The Child, And 2) Disabled Adults With A Certificate: A) With A Significant Degree Of Disability Or B) With A Moderate Degree Of Disability, Or C) Treated On An Equal Footing With The Judgments Referred To In Point (A). A And B, In Accordance With Art. 5 And Art. 62 Of The Act Of August 27, 1997 On Vocational And Social Rehabilitation And Employment Of Disabled People (Journal Of Laws Of 2025, Item 913 1.5. Personal Assistance Services Include Care For Disabled People, Mainly Those Who Are Lonely, In Performing Everyday Activities And Functioning In Social Life. 1.6. The Contractor Is Not Entitled To Reimbursement Of Travel Costs To And From The Place Of Service, Purchase Of Meals, As Well As Other Costs Incurred During The Provision Of The Service.1.7. The Contractor Will Keep Records Of The Purchase Of: A. Public/Private Transport Tickets For The Assistant Accompanying The Disabled Participant, B. Entry Tickets To Cultural, Entertainment, Sports Or Social Events, Etc. By The Assistant Accompanying The Participant.1.8. Personal Assistance Services Will Be Provided As Part Of The Daily Stay At The Place Of Residence Of The Disabled Person.1.9 Specified In The Personal Assistant For Persons With Disabilities Program For Local Government Units - 2026 Edition.
