1. The Subject Of The Order Is The Purchase, Delivery And Commissioning Of Medical Devices For Diagnosing, Monitoring And Saving Lives, As Well As Training Employees In The Use Of This Equipment. The Medical Equipment Offered By The Contractor Must Have Valid Permits For Use In Poland - In Accordance With The Act Of April 7, 2022. On Medical Devices (Journal Of Laws Of 2024, Item 1620, As Amended) And Have Appropriate Declarations Of Conformity (Ce Certificates). A Detailed Description Of The Subject Of The Order Is Included In Attachment. 1A-1 To 1A-4 To The Swz (Hereinafter Referred To As Annex 1A To The Swz). This Description Should Be Read With Any Changes Resulting, For Example, From The Answers Provided. Part No. 1 - Ultrasound Device - A Detailed Description Of The Subject Of The Order Is Included In The Annex. 1A-1 To Swz3. The Subject Of The Order Should Be Free From Physical And Legal Defects, Brand New (Year Of Production Not Older Than 2025 - In Accordance With The Requirements Specified For Each Package In Annex 1A), Not Previously Used By Third Parties, In Particular It Has Not Served As An Item Used For Demonstration Purposes And Is Of Good Quality. It Has Also Not Been Previously Reconditioned.4. The Order Is Carried Out As Part Of The Task Entitled: Implementation Of The Civil Protection And Civil Defense Program For 2025-2026 5. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Contract Template Constituting Annex No. 3 To The Siwz.6. Medical Equipment (Which Has Been Classified As A Medical Device) - Must Have Documents Allowing For Trade And Use In The Territory Of The Republic Of Poland In Accordance With The Act Of April 7, 2022. On Medical Devices (Journal Of Laws 2024, Item 1620, As Amended). 7. The Subject Of The Order Must Be Complete In Terms Of The Ordering Partys Requirements Specified In Annexes 1A-(1-4) (For Each Part) To The Tor And, After Being Handed Over To The Ordering Party, Ready To Undertake Medical Activities. All Requirements Specified In The Documents Indicated Above Are Minimum Requirements And Their Fulfillment Is Obligatory. Failure To Meet The Above-Mentioned Minimum Requirements Will Result In Rejection Of The Offer As Inconsistent With The Terms Of The Contract Pursuant To Art. 226 Section 1 Point 5 Of The Public Procurement Law 8. After Handing Over The Subject Of The Order For Ongoing Operation, Confirmed By The Acceptance Protocol By The Ordering Party, The Contractor Is Obliged To Conduct, At Its Own Expense, Training For The Ordering Partys Employees In The Scope Of Proper Operation, Maintenance And Inspection Of The Technical Condition Of Medical Equipment And Apparatus.
