1. The Subject Of The Order Is The Provision Of Shelter Services In A Homeless Shelter For 15 Homeless People (Women And Men) Pursuant To Art. 36 Point 2 Letter And, Art. 48A Of The Act Of March 12, 2004 On Social Assistance For Homeless Persons Indicated By The Ordering Party, Whose Last Place Of Residence Is The Boguszów-Gorce Commune - Based On Art. 101 Section 2 Of The Act Of 12 March 2004 On Social Assistance (Journal Of Laws Of 2025, Item 1214, As Amended). The Simultaneous Number Of People Using Shelter Services Depends On The Real Needs Of The Ordering Party And May Be Lower Or Higher Than Indicated In The Swz. In The Event Of A Lower Number Of People Requiring Shelter, The Ordering Party Provides For Payment For The So-Called Readiness. In The Event Of A Higher Number Of People Requiring Shelter, The Ordering Party Expects The Execution Of An Increased Order, In Accordance With The Contractors Capabilities.2. Shelter Is Granted On The Basis Of An Individual Administrative Decision Granting Assistance In The Form Of Shelter Issued By The Director Of The Social Welfare Center In Boguszów-Gorce Or Another Authorized Employee. It Is Allowed To Provide Services Based On Data Provided By Phone Or E-Mail.3. In Justified Cases, The Ordering Party May Demand Immediate Transport Of Persons To Whom It Provides Assistance In The Form Of Shelter, From Their Place Of Stay (In The Boguszów-Gorce Commune) Or From The Ordering Partys Seat To The Place Of Shelter. The Transport Request Applies To Situations In Which It Is Impossible To Immediately Reach The Place Of Shelter Using Available Public Transport. There Is No Additional Payment For Transport.4. A Shelter For Homeless People Provides Homeless People Who Have Signed A Social Contract With 24-Hour, Temporary Shelter And Services Aimed At Strengthening Social Activity, Ending Homelessness And Gaining Independence In Life. In Particular, The Minimum Scope Of Services Provided By The Contractor Is To Be Identical To The Minimum Standard Of Basic Services Provided In Shelters For Homeless People And The Minimum Standard Of Facilities In Which Shelters For Homeless People Are Located, Specified In The Regulation Of The Minister Of Family, Labor And Social Policy Of April 27, 2018 On Minimum Standards Of Night Shelters, Shelters For Homeless People, Shelters For Homeless People With Care Services And Heating Rooms (Journal Of Laws Of 2018, Item 896).
