1. The Subject Of The Order Is Provision Of Care Services In The Place Of Residence In Accordance With Article 50 Of The Act Of March 12, 2004 On Social Assistance For Persons Residing In The Municipality Of Ciężkowice - Beneficiaries Of The Municipality Social Welfare Center In Ciężkowice.2. Scope Of The Order: 1) The Scope Of The Order Includes The Provision Of Care Services In A Total Amount Of 7,440 Hours For 35 People In The Period From January 1, 2026 To December 31, 2026. 2) The Planned Average Number Of Hours Of Services Provided Per Month Is 620. 3) The Basis For Settlements With The Contractor Will Be 1 (One) Working Clock Hour (60 Minutes), Which Does Not Include Time Commuting/Access To The Patient.4) Services Are To Be Provided On Working Days, Holidays And Holidays. The Number Of Hours Of Services Provided Will Depend On The Scope Of Services, The Number Of Patients And Their Needs And Will Change During The Duration Of The Contract.5) The Contractor Will Start Providing Services For A Specific Person After Receiving From The Ordering Party A Catalog Of Care Services Or Specialized Care Services, Which Will Specify The Date Of Commencement Of The Provision Of Services, The Scope And Frequency Of Activities Performed As Part Of The Services.6) The Services Are To Be Provided In Accordance With The Catalog Of Services. In Particularly Justified Cases, When There Is A Sudden Deterioration Of Health Or When The Provision Of Services Results From The Need To Urgently Provide Appropriate Care, E.G. After Leaving The Hospital, The Contractor May Commence The Provision Of Services Based On A Telephone Or Oral Notification Made By The Ordering Party. The Catalog Of Services In Written Form Will Be Provided Immediately At A Later Date.7) The Services Will Be Ordered On The Basis Of A Job Card Issued By A Social Worker Employed At The Municipal Social Welfare Center In Więcwice.8) The Job Card Prepared On The Basis Of An Administrative Decision Granting This Type Of Services Will Contain The Following Data: Name And Surname, Contact Telephone Number (If Available) Of The Person To Whom The Services Will Be Provided, Possibly Also Details Of The Legal Guardian, Place Of Service Provision, Type Of Services, Scope Care Services And The Number Of Days Per Week, The Number Of Hours Per Day, The Number Of Hours Per Month, The Period For Which The Care Services Are Granted9) The Hours Of Providing The Services Do Not Include The Time It Takes For The Caregiver/Caregiver To Reach The Place Where They Are Provided And The Time Necessary For The Caregiver/Caregiver To Move Between The Beneficiaries10) Due To The Specificity Of The Subject Of The Order, The Ordering Party Reserves The Right To Significantly Change The Number Of People Using The Care Services During The Term Of The Contract And The Hourly Dimension Of The Services - Without The Possibility Of Claims This Title On The Part Of The Contractor, Among Others In Terms Of Changing The Price For
