The Ordering Party Requires That The Order Be Carried Out By A Team Of Doctors Consisting Of At Least Certifying Doctors In The Following Specializations: A) Pediatrics - 1 Person; B) Neurology - 1 Person; C) Orthopedics - 1 Person; D) Rheumatology - 1 Person; E) Internal Medicine – 1 Person;F) Surgery – 1 Person;G) Ophthalmology – 1 Person; 2. The Certifying Doctor Is Obliged To Have A Valid Certificate Issued In Accordance With § 23 Of The Regulation Of The Minister Of Economy, Labor And Social Policy Of July 15, 2003. On Assessing Disability And The Degree Of Disability (Journal Of Laws Of 2021, Item 857, As Amended) Or Committing To Undergo Training Under The Principles Set Out In § 21(2) Of This Legal Act; - The Chairman Of The Adjudicating Panel Is: 1) A Doctor With The Title Of A Specialist, Or 2) A Doctor With At Least The First Degree Specialization, Or 3) A Doctor Undergoing Specialization Training In A Specific Field Of Medicine, Who Has Completed The Relevant Basic Module For A Given Specialization Training Or Being In The Third Year Of This Training In The Case Of Specializations For Which A Uniform Training Module Has Been Provided - In Particular In The Following Fields Of Medicine: Internal Diseases, Pediatrics, Orthopedics And Traumatology Of The Musculoskeletal System, Neurology, Psychiatry, Ophthalmology And Otorhinolaryngology. The Chairman Of The Adjudicating Panel May Also Be A Person Practicing The Medical Profession For At Least Five Years - Excluding The Periods Of Practicing The Profession For The Purpose Of Completing Postgraduate Internship, Practicing A Profession On The Basis Of A Conditional Right To Practice A Profession, Practicing A Profession On The Basis Of A Right To Practice A Profession For A Specific Scope Of Professional Activities, Period And Place Of Employment In A Medical Entity. The Contractor May Have Doctors With A Greater Number Of Specializations, The Ordering Party May Appoint Adjudicating Panels Tailored To A Wider Range Of Applicants Diseases. The Selected Contractor Will Be Obliged To Submit To The Ordering Party A Current Certificate Issued In Accordance With § 23 Of The Regulation Of The Minister Of Economy, Labor And Social Policy Of July 15, 2003. And Documents Confirming Qualifications And The Right To Practice Medicine, Together With A List Of The Team Of Certifying Doctors - No Later Than On The Date Of Commencement Of The Order. The Team Of Certifying Doctors May Consist Of Doctors With More Than One Of The Above-Mentioned Specializations; Such A Doctor Will Be Able To Act As An Adjudicator In The Field Of Specializations He Has And Which Are Covered By The Subject Of The Order.
