Part 1: Provision Of Specialized Care Services For People With Mental Disorders From The Świlczaw Commune 20261. The Subject Of The Order Under Part No. 1 Is The Provision Of Specialized Care Services For People With Mental Disorders For The Residents Of The Municipal Social Welfare Center In Świlcza In The Place Of Residence Of These People On Working Days, Excluding Statutory Holidays And Holidays.2. Specialized Care Services Are Understood As Services Specified In The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Journal Of Laws No. 189, Item 1598, As Amended), I.E. Care Services Provided By People With Specialized Professional Training, Tailored To The Specific Needs Of People Requiring Help In This Form, Resulting From The Type Of Their Disease, Including Specialized Care Services For People With Mental Disorders.3. The Contractor Will Provide Services On The Basis Of Individual Decisions Issued By The Municipal Social Welfare Center In Świlcza As Well As Medical Documentation Provided To The Contractor After Signing The Contract.4. The Estimated Number Of Clients For Whom Specialist Care Services Will Be Provided For A Period Of Up To 12 Months, I.E. No Earlier Than January 1, 2026 And No Later Than December 31, 2026, Is: 1) In The Period From January 1, 2026 To July 31, 2026 - 7 People, 2) In The Period From August 1, 2026 To December 31 2026 – 10 People.5. The Estimated Total Number Of Hours For Specialist Care Services For People With Mental Disorders In The Period From January 1, 2026 To December 31, 2026 Is Approximately 147 Hours/Month (I.E. In The Period From January 1, 2026 To July 31, 2026 (A Total Of 1,029 Hours). From August 1, 2026 Until December 31, 2026, On Average 200 Hours/Month For 5 Months, I.E. On Average 1,000 Hours. The Total Number Of Hours - 2,029.6. Before Signing The Contract, The Contractor Will Be Obliged To Submit To The Ordering Party A List Of Persons Intended To Provide The Services That Are The Subject Of This Order And Copies Of Documents Confirming The Professional Qualifications, Internship And Experience Of These Persons In Accordance With The Regulation Of The Minister Of Social Policy Of September 22, 2005 Regarding Specialized Care Services (Journal Of Laws No. 189, Item 1598, As Amended). 7. The Estimated Cost Of One Hour Of Remuneration For The Provision Of A Specialized Service Is Pln 59.15 Gross. A Detailed Description And Scope Of The Subject Of The Order Is Specified In The Terms Of Reference.
