The Subject Of The Order Is The Provision Of Specialized Care Services In The Field Of Physical Rehabilitation And Improvement Of Impaired Body Functions To The Ordering Partys Clients, At Their Place Of Residence, To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004 On Health Care Services Financed From Public Funds (Consolidated Text: Journal Of Laws 2024.146), With Particular Emphasis On The Ndt-Bobath Method, In Accordance With The Recommendations Of Doctors Or Specialists. In The Field Of Exercise Rehabilitation Or Physiotherapy.2. The Subject Of The Order Will Be Provided At The Place Of Residence Of The Ordering Partys Charges, Located Within The Administrative Boundaries Of The Ochotnica Dolna Commune, On Working Days, Saturdays And Holidays, At Hours Previously Agreed With The Ordering Partys Charges.3. The Services Will Be Provided In Accordance With The Provisions Contained In The Draft Provisions Of The Contract (Constituting Annex No. 1 To This Specification) And The Applicable Regulations, Recommendations And Procedures In This Regard, And In Particular In Accordance With The Requirements And Conditions Arising From: 1) The Act Of March 12, 2004 On Social Assistance (Journal Of Laws Of 2023, Item 901, As Amended); 2) The Act Of September 25 2015 On The Profession Of Physiotherapist (Journal Of Laws Of 2023, Item 1213, As Amended);3) Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialist Care Services (Journal Of Laws Of 2005, No. 189, Item 1598, As Amended);4) Other, Relevant From The Point Of View Of The Subject Order, Generally Applicable Regulations.4. The Services Will Be Provided From January 1, 2026 To December 31, 2026, But No Longer Than Until The Funds Allocated By The Ordering Party To Finance The Order Are Exhausted.5. It Is Assumed That During The Entire Period Of Provision Of The Services Covered By The Order, Up To 15 Clients Will Use These Services, In A Total Amount Of: 34.5 Hours Per Week, Which Constitutes A Total Of 1,794 Hours During The Entire Period Of The Order Implementation (Which Number Of Hours Is The Maximum Number Of Hours Of Provision Of Services By The Contractor During The Entire Period Of The Contract), Whereas: 1) For One Hour Of Provision Of Services, The Ordering Party Accepts One Clock Hour (60 Minutes) Of Providing The Services Covered By The Subject Of The Order In The Place Of Residence Of The Client.
