The Subject Of The Order Is To Provide 24-Hour Temporary Shelter With Full Board And The Provision Of The Necessary Social Conditions And Care Services To People In A Homelessness Crisis Referred By The Social Welfare Center, Who, Due To Age, Illness Or Disability, Require Partial Care And Assistance In Meeting Their Necessary Life Needs, In Accordance With Art. 48A Of The Act Of March 12, 2004 On Social Assistance (Journal Of Laws Of 2025, Item 1214, As Amended). B) Service Provision Period: From January 1, 2026. Until December 31, 2026, 7 Days A Week. C) The Contractor Must Have At Least 4 Years Of Experience In Running: A Shelter For Homeless People With Care Services. D) The Contractor Is Obliged To Indicate In The Offer The Place Where The Service Will Be Provided. The Ordering Party Reserves The Right To Verify The Indicated Place, In Terms Of Preparation For The Implementation Of The Ordered Service, Before Concluding The Contract And Throughout The Entire Order Execution Period. E) The Contractor May Indicate In The Offer A Shelter Located A Maximum Of 150 Km From The Ordering Partys Headquarters. F) The Contractor Undertakes To Transport Homeless People From Their Place Of Current Stay, I.E. From The Fundacja Leśne Zacisze Shelter, Sokowy Kąt 1, 09-135 Siemiątkowo To The Shelter The Contractor With Appropriate Transport Adapted To Transport People To The Extent Appropriate To The Persons Current Health Condition And Dysfunction. The Organization And Cost Of Transport Are The Responsibility Of The Contractor And Must Be Included In The Offer Price Of The Order. Currently In The Above-Mentioned 4 People Are Staying In The Shelterg) The Expected Number Of Homeless People Covered By The Service Is 10 People. Due To The Specificity Of The Order, Which Is Characterized By Variability Of Customer Needs Due To Their Changing Health Condition And Personal Situation, The Number Of Homeless People Provided By The Ordering Party Is The Number Expected Throughout The Duration Of This Order. The Ordering Party Reserves The Right To Reduce The Above. Number Of People Depending On The Actual Needs In This Regard. The Contractor Declares That He Accepts The Above Reservations And Will Not Be Entitled To Any Claims, Including Monetary Ones, Against The Ordering Party. H) Payment Of Remuneration To The Contractor Will Be Made By The Ordering Party Only For People Who Are Currently Staying In The Shelter (Depending On The Number Of Days Of Their Stay In The Contractors Shelter).
