The Subject Of The Order Is The Provision Of Services Consisting In Confirming Death, Determining Its Cause By Inspection And Issuing Death Certificates To Persons Who Died In Public And Non-Public Places In The Administrative Area Of ​​The City Of Mysłowice, In Accordance With Art. 11 Section 2 Of The Act Of January 31, 1959 On Cemeteries And Burial Of The Dead (Consolidated Text: Journal Of Laws Of 2025, Item 637) And The Regulation Of The Minister Of Health And Social Welfare Of August 3, 1961 On The Certification Of Death And Its Causes (Journal Of Laws Of 1961, Items 39 And 202).1.1. Description Of The Subject Of The Order: A) The Provision Of Services Will Be Of An Incidental Nature And Will Apply Only To Cases Where It Is Impossible To Determine The Primary Care Physician Or The Doctor Treating The Deceased In The Last Illness, And The Death Was Not Confirmed By Authorized Persons Called In The Event Of An Accident Or Sudden Illness. B) The Subject Of The Order May Only Be Performed By An Entity With Documented Professional Qualifications And Authorizations Necessary To Perform It, In Accordance With Applicable Legal Provisions In This Regard, Or An Entity With Persons Meeting The Above-Mentioned Conditions. C) The Contractor Is Entitled To Perform The Task Personally Or With The Help Of Persons With Appropriate Qualifications, For Whose Actions And Omissions He Is Responsible As For His Own Actions And Omissions. E) The Contractor Of The Task Carries Out The Subject Of The Order After Telephone Information From The Municipal Crisis Management Center In Mysłowice, Ul. Mikołowska 50 Tel. 47 851 09 40.F) The Contractor Provides 24-Hour Telephone Communication, Every Day Of The Year, In Order To Receive Notifications Sent By The Municipal Crisis Management Center In Mysłowice About The Need For An Authorized Person To Arrive At The Place Where The Body Is Located.G) The Contractor Or Persons With The Help Of Whom The Subject Of The Order Is Performed, After Receiving The Notification, Are Obliged To Arrive At The Place Of The Incident On Their Own, Without Undue Delay, Up To 6 Hours From Receiving The Request, In Order To Complete The Task.
