2.2 Detailed And Quantitative Scope Of The Subject Of The Order: It Is Planned To Provide Care Services In A Total Number Of Approximately 11,520 Full Clock Hours In The Period From January 1, 2026. Until December 31, 2026, In The Żary Commune. 2.3 The Above Service Will Be Provided Under The Following Conditions: A) The Order Covers The Provision Of Care Services In The Amount Of Approximately 11,520 Hours, Including: Care Services In The Amount Of Approximately 11,000 Hours, Specialized Care Services In The Amount Of Approximately 260 Hours And Specialized Care Services For People With Mental Disorders In The Amount Of Approximately 260 Full Clock Hours In The Period From January 1, 2026. Until December 31, 2026 (12 Months) At The Place Of Residence, In The Żary Commune, To Persons Who, Due To Chronic Illnesses, Age Or Disability, Require The Help Of Another Person, B) The Number Of Hours Of Care Services Specified In The Swz Is An Estimate And May Be Increased Or Decreased, Provided That The Total Value Of Changes In The Gross Price Of The Subject Of The Order Does Not Exceed 20%. Changes In The Number Of Hours As Well As A Decrease Or Increase In The Value Of The Contract Do Not Require An Annex To The Contract. The Ordering Party Emphasizes That The Number Of Hours Of Services Provided Should Be Understood Only As The Actual Time Of Providing Services Without Preparatory Activities, E.G. Travel To The Client, C) The Contractor Declares That It Will Not Pursue Financial Claims For A Reduced Number Of Care Services Provided, D) The Number Of Hours Of Care Services Depends On The Needs Of Residents, E) Care Services Should Be Adapted To The Specific Needs Resulting From The Type Of Illness Or Disability. The Contractor Should Have Staff To Carry Out This Order With Qualifications To Perform The Profession Of Community Carer, Medical Carer, Carer, Carer For The Elderly Or Have Completed Training/Course In The Scope Covered By The Subject Of The Order, I.E. Completed Training/Course For Carers Of The Elderly And/Or Disabled People, Home Carers, Carer In A Social Welfare Home. The Ordering Party Specifies That The Persons Providing The Services Should Be Employed By The Contractor In The Form Of An Employment Contract. Moreover, To Coordinate This Task, It Is Necessary To Employ A Person/People In The Form Of An Employment Contract. As Part Of Its Service, The Contractor Will Provide A Specific Standard Of Service Regarding Available Contact Channels, I.E. Having An Office In The City Or Commune Of Żary, Open At Least 4 Hours A Day, At Least Once A Week, Which Will Be Assessed As Part Of The Offer Evaluation Criteria.
