1. The Subject Of The Order Is The Successive Delivery Of Milk And Dairy Products For The Needs Of The Social Welfare Home In Zawiercie In The First And Second Quarter Of 2026.2. The Detailed Type Of Assortment And Scope Of The Order Was Specified In The Assortment And Price Form, Which Constitutes Annex No. 1.1 To The Swz. The Quantities Of Individual Goods Given In This Form Are The Quantities Expected By The Ordering Party, Which Were Adopted In Order To Compare Offers And Select The Most Advantageous Offer. The Ordering Party Reserves The Right To Change The Quantity Of Individual Goods Depending On Current Needs While Maintaining Unit Prices And Up To The Amount Of The Future Public Procurement Contract And The Right To Limit The Value Of The Order By A Maximum Of 30%.3. The Goods Covered By This Order And The Method Of Their Delivery Must Comply With The Provisions Of The Specification Of Order Conditions, The Assortment And Price Form And The Applicable Standards And Certificates As Well As The Legal Provisions Applicable To The Subject Of The Order.4. The Place Of Delivery Is The Registered Office Of The Ordering Party: Social Welfare Home In Zawiercie, Ul. Rzemieślnicza 9, 42-400 Zawiercie.5. The Execution Of The Order Must Be Consistent With The Haccp System Principles Implemented And Applied At The Contractors Plant.6. The Contractor Will Deliver The Goods By Means Of Transport Adapted To Transport Food In Accordance With Applicable Regulations, Under Pain Of Non-Acceptance Of The Delivery By The Contractor.7. At Each Written Request Of The Ordering Party, The Contractor Will Enable The Means Of Transport And Containers For Transporting Goods To Be Inspected For Cleanliness.8. Upon Each Written Request, Within The Deadline Specified By The Ordering Party, The Contractor Will Present (If Applicable): 1) Decision Of The State District Sanitary Inspector For The Means Of Transport, Which Allows The Transport Of Milk And Dairy Products, 2) Current Medical Certificate From The Examination For Sanitary And Epidemiological Purposes Of The Person Delivering The Goods.9. Deliveries Will Be Made Successively, Depending On The Current Needs Of The Ordering Party, On The Basis Of Separate Orders Specifying Min. Quantity, Product Range And Delivery Date.10. The Ordering Party Requires That Deliveries Be Made On Working Days, Five Times A Week.11. The Contractor Will Ensure Deliveries Between 6:00 A.M. And 7:00 A.M. On The Date Specified In The E-Mail By The Ordering Partys Representative. This Deadline Cannot Be Shorter Than 2 Business Days, And The Delivery Day Cannot Be A Public Holiday.12. The Remaining Requirements For The Contractor, As Well As The Method And Detailed Conditions For The Implementation Of The Subject Of The Order, As Well As The Terms Of Payment, Are Specified In The Draft Provisions Of The Future Contract Constituting Annex No. 2 To The Swz.
