1. The Subject Of The Order Is The Supply Of Dental Materials And Instruments For The Lower Silesian Medical Center Dolmed S.A.2. The Execution Of The Order Includes Transport To The Registered Office Of The Ordering Party (Wrocław, Ul. Legnicka 40).3. The Offered Products Must Be Approved For Trading And Use Under Currently Applicable Legal Regulations.4. Product Expiry Date – Min. 12 Months From The Delivery Date Or Free Replacement Before The Expiry Date.5. The Content Of The Contractors Offer Must Correspond To The Content Of The Specifications Of The Contract Terms. 6. The Detailed Subject Of The Order Is Included In The Assortment And Price Form, Which Constitutes Annex No. 2 To Swz.7. Small Quantitative Shifts Between Individual Product Items Are Allowed, Within 30% Of The Net Value Of The Subject Of The Order, While Maintaining The Overall Value Of The Subject Of The Contract.8. Partial Deliveries: 3-4 Times A Month, According To The Customers Needs9. Maximum Waiting Time For Partial Delivery: Up To 5 Business Days From Placing The Order10. Deliveries Will Be Made At The Contractors Expense For Partial Orders Exceeding Pln 300 Gross.11. The Order Must Be Completed In Accordance With The Ordering Partys Requirements Specified In The Swz And Published Explanations.12. The Contractors Offer Must Be Clear And Comprehensive, I.E. It Must Cover The Entire Subject Of The Order.13. Any Indication Of Trademarks, Patents Or Origin, Source Or Specific Process That Characterizes The Products Or Services Provided By A Particular Contractor That May Appear In The Swz Is Intended Only To Determine The Standard And Should Be Treated As Examples Only. The Listed Materials And Products May Be Replaced By Other, Equivalent Ones, Which The Contractor Is Obliged To Indicate In The Submitted Offer. If Equivalent Solutions Are Used, The Contractor Is Obliged To Demonstrate That They Meet The Requirements Specified By The Ordering Party, In Particular In Terms Of The Use (Purpose) Of The Products, Their Size, Shape And Composition Described In Annex No. 2 To The Swz.14. Guarantee And Warranty:1) Details Regarding The Warranty And Warranty Are Specified In The Draft Provisions Of The Contract (Template Contract).2) The Warranty Period Begins After The Protocol Acceptance Of The Subject Of The Contract (Without Defects).
