1. The Subject Of The Order Is The Delivery, Installation And Commissioning Of A Set Of Endoscopic Devices And Medical Equipment For The Needs Of Powiatowe Centrum Zdrowia Sp. Z O. O. In Otwock, Including: 1) Electrosurgical Platform With The Function Of Closing Vessels Up To 7 Mm And The Possibility Of Cutting And Coagulation Using The Monopolar And Bipolar Method - 1 Pc; 2) Endoscopic Column (Endoscopic Tower) With An Image Processor, Light Source, Monitor, Endoscopic Trolley And Accessories - 1 Pc; 3) Automatic Endoscope Washing Station, Including A Single-Chamber Washer And Reprocessors - In Accordance With Annex No. 2A - 2 Pcs.;4) Set Of Laparoscopy/Hysteroscopy Tools, Including A 17.5/18.5 Ch Bipolar Resectoscope With Optics And Accessories - 1 Pc.;5) Steam Sterilizer (1 Ste) - 1 Pc.;6) Vision Track - Led Light Source With Optical Fiber - 1 Pc.2. The Delivery Also Includes: 1) Assembly And Installation Of All Devices, 2) Commissioning, 3) Performance Of Verification Tests, 4) Training Of The Ordering Partys Staff, 5) Provision Of Full Documentation Of The Devices.3. All Equipment Must Be Brand New, Complete, Unused, Not For Exhibition, With A Year Of Manufacture Not Older Than 2024, Ready For Immediate Use After Installation.4. All Devices Must: 1) Meet The Requirements Of Relevant Standards, In Particular Regarding Medical Devices, Safety And Electromagnetic Compatibility; 2) Be Marked In Accordance With The Provisions Of The Act On Medical Devices; 3) Have Declarations Of Conformity/Ce (If Applicable); 4) Comply With The Minimum Parameters Indicated In Annex 2A - Required Technical Parameters. 5. The Contractor Will Ensure The Delivery Of All Elements, Accessories, Cables, Adapters And Equipment Necessary For Full Functionality And Cooperation Of All Devices.6. The Delivery Includes Training Of The Ordering Partys Medical And Technical Staff In The Operation, Operation And Basic Maintenance Of The Ordered Devices (Confirmed By The Protocol).7. The Contractor Will Provide Complete Documentation: Instructions In Polish, Warranty Cards, Test Reports, Installation Diagrams And Other Required Documents.8. The Contractor Will Provide Warranty And Post-Warranty Service In Accordance With The Requirements Specified In Annex No. 2A.9. The Contractor Is Fully Responsible For The Quality, Completeness And Compliance Of The Delivered Equipment With The Description Of The Subject Of The Order.10. A Detailed Description Of The Subject Of The Order Is Included In This Tor And: Annex No. 2 - Assortment And Price Form; Annex No. 2A - Required Technical Parameters; Annex No. 3 - Draft Contract.
