1. The Subject Of The Order Is The Provision Of Living Services For Residents In Supported Housing At The Social Support Center At Ul. Ogrodowa 2A In Wałbrzych, In Accordance With Art. 53 Section 5 Of The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2025, Item 1214, As Amended) And § 4 Of The Regulation Of The Minister Of Family And Social Policy On Training And Supported Housing Of October 30, 2023 (Consolidated Text: Journal Of Laws Of 2023, Item 2354).The Ordering Party Orders A Service Consisting In The Provision Of Living Services In Supported Housing. Living Services Consist In Assistance In Performing Activities Necessary In Everyday Life And Social Contacts, In Order To Maintain And Develop A Persons Independence At The Level Of His Or Her Psychophysical Capabilities. The Minimum Standard Of Services Provided In A Sheltered Supported Apartment Includes Assistance In: 1) Performing Activities Necessary In Everyday Life Regarding: A) Moving Around, B) Maintaining Personal Hygiene, Including Care And Personal Hygiene Of A Resident Who Cannot Do It On Their Own (Including Dressing, Changing Underwear And Bed Linen, Pampering And Personal Toilet), Anti-Decubitus Prevention (Changing Positioning, Patting The Back), C) Administering Medications Recommended By A Doctor, D) Cleaning, Including Keeping Clean: Rooms (Including Windows And Doors) And Sanitary Facilities, Common Communication Areas, I.E. Corridor, Canteen, Common Toilet For The Disabled, Conference Room, E) Making Purchases At The Residents Request With His Or Her Money, Depending On Individual Needs, F) Preparing Meals, Taking Into Account The Diet And, If Necessary, Providing Assistance In Eating Them, G) Dealing With Personal Matters (Including Official Ones), H) Maintaining Individual Interests (E.G. Delivering Newspapers, Books, Organizing Walks), In Accordance With The Needs Of The Resident - Within The Scope Of The Written Arrangements, I) In Justified Cases, Assisting The Contractors Employee During The Residents Medical Visit, 2) Implementing Social Contacts By Enabling The Person To: A) Maintain Family Ties, B) Participate In The Life Of The Local Community. The Contractor Will Be Obliged To Ensure The Possibility Of Providing Living Services To 23 Residents Seven Days A Week, From 7.00 A.M. To 7.00 P.M., Including On Holidays And Public Holidays Work. On Average, The Number Of Hours Of Provided Household Services Per Month Will Amount To Approximately 1,782.5 Clock Hours, Including Public Holidays - In Accordance With The Act Of January 18, 1951 On Non-Working Days (Consolidated Text: Journal Of Laws Of 2025, Item 296). The Maximum Number Of Hours Of Service During The Entire Duration Of The Contract Is 64,170. The Given Number Of Hours Is The Maximum, And The Number Of Hours Of Providing Residential Services Actually Assigned To The Contractor By The Ordering Party Will Result From The Actual Needs Of The Residents.2.
