Tenders are invited for All Other Repairs Other than the above Repairs Instructions and Conditions 1. Suppliers can register for more than one item. 2. Should show the classification of special items of principals regarding medical supplies (Example: Suture, Mesh, Laparoscopy Instrument, Orthopedic Items) 3. Self-prepared application as per the following specimen along with a copy of the Business Registration Certificate and receipt for the payment can either he sent by Registered Post addressed to Chairman, Regional Procurement Committee, District General Hospital, Nawalapitiya to be received at or before 11.30 a.in. on 11.12.2025 or may be hand delivered and deposited in the Tender Box placed at the Head Office of this Institution. 4. Top left-hand corner of the cover enclosing the application should be marked as Registration of Suppliers for the Year 2026. 5. Must be agreeable to Supply Goods and Services on 06 weeks credit basis. 6. Quotations are called from registered Suppliers for Goods and Services but the Director of the Hospital reserves the right of calling applications from unregistered external Suppliers as well and the Director reserves the right of rejection of any application sent for registration without assigning any reason whatsoever. 7. Payments will be made only after the completion of the services and supply of all the materials to the stores. 8. Transport facilities should be provided free of charge. 9. Institutions that fail to respond when quotations are called for or that do not supply goods according to the quoted prices will be blacklisted. 10. A non-refundable Registration Fee of Rs. 1000.00 should be paid for each item to Shroff Division of this hospital and the receipt or copy of the receipt should be submitted along with the application. Cheques and Money Orders will not be accepted. If registration is sought for number of items, separate applications should be submitted for each and every item and the copy of the Business Registration Certificate and the copy of the receipt of the Registration Fee paid should be annexed. All applications should be forwarded enclosed in a single cover. 11. When obtaining abovementioned Goods and Services, attention will be paid to the recently issued instructions and guidelines of the National Procurement Agency. 12. If any supplier has not registered as a supplier to the government with www.promise.lk website as yet, they should take steps to register.
