The Ordering Party Requires That The Contractor Has A Facility Providing 24-Hour Shelter For Homeless People Located At Such A Distance - Counting From The Administrative Boundaries Of The City Of Pruszcz Gdański - Where The Average Travel Time By Passenger Car Is No More Than 1 Hour (Based On The Contractors Declaration). The Cost Of Transporting A Homeless Person From Pruszcz Gdański To The Shelter Is Covered By The Contractor. The Contractor Running A Shelter For Homeless People With Care Services, Providing Services Under The Order, In Addition To The Provisions Specified In The Contract Template: 1) Keeps A Register Of People Using The Facilitys Assistance Referred By The Municipal Social Welfare Center In Pruszcz Gdański, 2) Conducts Social Work, In Particular Motivating Homeless People To Take Action To Solve Difficult Life Situations, 3) Motivates Homeless People To Take Up Activity And Independence In Life And Work And Prevents Social Exclusion, 4) Ensures Appropriate Support And Assistance To People Who Require It, In Particular Due To Age, Disability, Long-Term Illness, Including The Possibility Of Undertaking Addiction Treatment,5) Provides Three Meals A Day (Including One Hot Meal) To Homeless People Who Require Increased Care Due To Age Or Health Condition In A Place Designated For This Purpose,6) If Possible - Changes Clothes At The Point Of Replacement For Clean Ones And Allows Dirty Laundry To Be Washed,7) Provides Contact With A Social Worker, As Well As Access To Information About Possible Forms Of Help, As Well As Basic Legal, Medical And Housing,8) Provides Basic Personal Hygiene Products In An Amount Enabling Maintenance Of Cleanliness,9) Provides Assistance To The Facilitys Residents In Life- Or Health-Threatening Situations By Enabling First Contact With A Doctor Or Health Care Worker,10) Maintains Constant Cooperation With A Social Worker Of The Municipal Social Welfare Center In Pruszcz Gdański.
