The relevant application forms could be obtained from the Accounts Division of this hospital on working days of the week from 17.11.2025 until 09.12.2025 between 9.00 a.m. and 3.00 p.m. by submitting the receipt obtained after paying the non-refundable fee of Rs. 1,000/- to the Shroff of Ampara General Hospital as Registration Fee for each category (for each supply/service).
