The Subject Of The Order Is The Provision By The Contractor To The Ordering Party Of Services In The Field Of Successive Collection, Transport And Disposal Of Medical Waste Generated At The Ordering Partys Headquarters.2. The Scope Of The Contract Includes The Collection, Transport And Disposal Of The Following Medical Waste, In Accordance With The Catalog Specified In The Regulation Of The Minister Of Climate Of January 2, 2020 On The Waste Catalog (Journal Of Laws Of 2020, Item 10): A) 18 01 02* - Body Parts And Organs As Well As Blood Containers And Preservatives Used For Its Storage (Excluding 18 01 03*) - Estimated 5,500 Kg, B) 18 01 03* - Other Waste Containing Live Pathogenic Microorganisms Or Their Toxins (Excluding 18 01 80 And 18 01 82) - Estimated 168,670 Kg, C) 18 01 04 - Waste Other Than Those Mentioned In 18 01 03 - Estimated 45,000 Kg,D) 18 01 06* - Chemicals, Including Chemical Reagents Containing Hazardous Substances - Estimated 15 Kg, E) 18 01 08* - Cytotoxic And Cytostatic Drugs - Estimated 30 Kg, F) 18 01 09 - Drugs Other Than Those Listed In 18 01 08 - Estimated 30 Kg. The Total Estimated Amount Of Waste Is 219,245 Kg Over A Period Of 33 Months.3. The Indicated Quantities Are Estimates. If The Ordering Party Fails To Provide The Amount Of Waste Specified In The Price Form During The Contract Period (33 Months), The Contractor Shall Not Be Entitled To Any Claims Against The Ordering Party.4. If The Transport Permit Or Waste Disposal Permit Expires, The Contractor Is Obliged To Immediately Submit A New Permit/Permit To The Person Responsible For The Collection Of Medical Waste, Under Pain Of Withdrawal From The Contract For Reasons Attributable To The Contractor.5. Waste Collection Will Be Carried Out Three (3) Times A Week, On Monday, Wednesday And Friday From 7:00 A.M. To 1:00 P.M..6. The Contractor Will Collect Waste From The Ordering Party Using Its Own Transport At Its Own Expense And Risk, With Loading And Unloading Using Its Own Forces. The List Of The Contractors Vehicles Is Included In Annex No. 2 To The Contract.7. Each Time When Collecting Waste For Disposal, The Contractor Will Measure Its Weight In The Presence Of The Ordering Partys Representative. 8. As Part Of The Contractual Remuneration, The Contractor Is Obliged To Insert And Install A Scale With An Appropriate Accuracy Class, Enabling Weighing Of Waste At The Ordering Partys Location In Accordance With Annex No. 1.A. The Contractor Will Prepare And Provide The Ordering Party With A Scale Installation Report, Including: Model, Production Date, Serial Number Of The Device And A Current Legalization Certificate. B. In The Event Of A Replacement Of The Scale Or The Legalization Deadline, The Contractor Will Immediately Submit A New Report And Certificate.C. The Contractor Bears Full Responsibility For The Operation Of The Device, Its Service, Repair And Re-Verification.9.
