1. The Subject Of The Order Are Preventive Examinations (Initial, Periodic And Control) Of Employees Of The Wielkopolska Regional Branch And Arimr District Offices. 7. The Subject Of The Order Is Also The Obligation Of The Doctor Providing Preventive Health Care To Employees To Participate (Once A Quarter) As A Member In The Work Of The Health And Safety Committee Appointed By The Ordering Party. 8. Moreover, The Subject Of The Order Is For The Doctor Providing Preventive Health Care To Review The Working Conditions Of Employees, Depending On The Needs And Recommendations Of The Doctor. The Inspections Will Be Carried Out Together With The Ordering Partys Employee Responsible For Health And Safety, And The Cost Of Transport To The Indicated Locations Is Borne By The Ordering Party. 9. The Subject Of The Order Is Also Protective Vaccinations - Against Tetanus And Against Ticks - For 60 Employees Of The Ordering Party And Sanitary And Epidemiological Tests: With Stool Testing For Carrier Status For 20 Employees And Without Stool Testing For 40 Employees Of The Ordering Party. After The Examination, The Occupational Medicine Doctor Is Obliged To Issue To The Employer A Medical Certificate From The Examination Carried Out For Sanitary And Epidemiological Purposes. 10. If An Employee Of The Ordering Party Does Not Have A Sanitary And Epidemiological Booklet, The Contractor Is Obliged To Provide Such A Booklet To The Employee Along With The Entry Made After The Tests, Identical To The Issued Decision.
