1.1. The Subject Of The Order Is The Provision Of Care Services For Clients Of The Social Services Center In Czeladź, Performed In The Place Of Residence Of These People, In Accordance With Applicable Regulations, In Particular The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2025, Item 1214, As Amended) And The Implementing Regulations Issued On The Basis Of The Above-Mentioned Act And In Accordance With The Requirements Specified In This Chapter Swz.Care Services Include Assistance In Meeting Everyday Life Needs, Hygiene Care, Care Recommended By A Doctor And, If Possible, Ensuring Contacts With The Environment - Art. 50 Section 3 Of The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2025, Item 1214, As Amended). Care Services Will Be Provided From Monday To Saturday, Depending On The Reported Needs Of People Using The Services, In The Czeladź Commune. The Scope Of Care (Economic) Services Includes The Following Tasks And Activities: 1. Providing Assistance In Meeting Everyday Life Needs In The Field Of Economic Services:2. Hygiene Care:3. Care Recommended By The Doctor:4. Ensuring Contacts With The Environment:5. The Contractor Is Obliged To Settle The Clients Financial Resources At The Disposal Of The Person Providing The Services, Including Keeping Records Of Expenses And Settling Accounts With Clients On The Money Spent, Based On Bills/Receipts.6. The Contractor Ensures That Services Are Performed By Persons With Appropriate Professional Preparation/Authorizations. To Provide Care Services, The Contractor Is Obliged To Provide Physically And Intellectually Fit Staff Who Take Care Of Personal Hygiene And Cleanliness And Have The Ability To Maintain Interpersonal Contacts. All Persons Whom The Contractor Directs To Carry Out The Subject Of The Order Must Have A Current Clean Criminal Record Certificate And Documentation Confirming Preparation For The Profession/Licenses Performed. The Contractor Will Be Obliged To Immediately Inform The Ordering Party About Changes In The Staff Employed, In Relation To The Staff Listed In The Offer (Coordinator) And To Provide The Ordering Party With Documentation Confirming His Professional Experience. The Contractor Is Obliged To Show The Ordering Party Documentation Confirming The Qualifications Of The Staff Shown In The Offer And To Provide The Ordering Party With An Up-To-Date List Of Employees At Each Request. Due To The Limited Number Of Characters, A Detailed Description Of The Subject Of The Order Is Included In The Terms Of Reference.
