Operating-Theatre Devices – Fn In Motola – Electroporation System. The Subject Of This Public Contract Is The Delivery And Installation Of An Electroporation System (Hereinafter Referred To As The Device) For The Department Of Imaging Methods Of The 2Nd Faculty Of Medicine Of The Uk And The Motol Faculty Of Medicine. The Subject Of The Performance Is Described In Detail In The Draft Purchase Contract For The Device, Which Forms Annex No. 3 Of The Procurement Documentation (Hereinafter Also Referred To As The Ks For The Device). The Offered Device Must Meet The Technical Requirements For The Subject Of Performance Established By The Client, To The Extent Specified In Annex No. 1 Of The Purchase Contract - Technical Specification Of The Subject Of Performance. The Device Must Be New, Unused, Delivered With All Accessories, Without The Need To Purchase Additional Components. The Scope Of Performance Also Includes: (I.) Installation Of The Device And Its Commissioning, Including Verification Of Its Functionality, Performance Of All Prescribed Examinations And Tests. (Ii.) Briefing/Training Of Medical Personnel Will Be Carried Out By A Person Authorized To Do So Pursuant To § 41 Of Act No. 375/2022 Coll., On Medical Devices And In Vitro Diagnostic Medical Devices (Hereinafter Referred To As Act No. 375/2022 Coll.). (Iii.) Warranty Service Of The Device, Which Will Be Carried Out By A Person Authorized For This Activity, Based On The Manufacturers Certificate Of Service Technician Training. The Contractual Relationship Between The Client And The Supplier Will Be Governed By The Purchase Agreement, Which Will Be Concluded With The Selected Supplier After Proving That All Conditions Required By The Client Have Been Fulfilled. The Scope Of Delivery Also Includes Post-Warranty Service Of The Device. The Contractual Relationship Between The Client And The Supplier Will Be Governed By The Agreement On The Provision Of Service Services (Hereinafter Also Referred To As The Service Contract), Which Will Be Concluded With The Selected Supplier After Proving That All Conditions Required By The Client Have Been Fulfilled. The Service Contract Is Attachment No. 4 Of The Procurement Documentation. Part Of The Subject Of Performance Is The Continuous Supply Of Electrodes For The Ablation Treatment Of Tumors (Hereinafter Referred To As Szm) For The Needs Of The Motol Medical Center, To The Extent According To Annex No. 1 Of The Purchase Agreement - List Of Goods With Unit Prices. (Hereinafter Also Referred To As Ks Na Szm), Which Forms Annex No. 5 Of The Procurement Documentation.
