The Subject Of The Order Is The Provision Of Occupational Medicine Services, Preventive Examinations - Initial, Periodic And Follow-Up For Employees Of The Wielkopolska Provincial Office In Poznań And The Branches In Kalisz, Konin, Leszno And Piła For 2026 And 2027 According To The Following Parts: 1) Part I - Provision Of Occupational Medicine Services, Preventive Examinations - Preliminary, Periodic And Follow-Up For Employees Of The Wielkopolska Office Provincial In Poznań;2. A Detailed Description Of The Subject Of The Order Is Included In Annex 4 To Swz.3. The Scope And Frequency Of Preventive Examinations Are Determined Each Time By The Doctor Performing The Preventive Examinations. Therefore, The Final Decision On The Scope And Frequency Of Tests Is Made By The Occupational Medicine Doctor.4. The Ordering Party Guarantees The Execution Of The Order In The Amount Of Min. 60%. The Contractor Is Not Entitled To Any Claims For Failure To Complete The Entire Order.5. The Ordering Party, In Accordance With § 6 Point 4 Of The Regulation Of The Minister Of Health And Social Welfare Of May 30, 1996 On Medical Examinations Of Employees, The Scope Of Preventive Health Care For Employees And Medical Certificates Issued For The Purposes Provided For In The Labor Code (Journal Of Laws Of 2023, Item 607), Requires The Participation Of The Doctor Providing Health Care For Employees In The Occupational Health And Safety Committee Operating At The Workplace - Applies Part I.6. The Services Will Be Provided Pursuant To Art. 4 Of The Act Of July 19, 2019 On Ensuring Accessibility For Persons With Special Needs (Journal Of Laws Of 2024, Item 1411, As Amended), Taking Into Account The Minimum Requirements Referred To In Art. 6 Above Act.7. The Ordering Party Provides For The Requirements Referred To In Art. 95 Section 1 Of The Act And Defines Them, Pursuant To Art. 281 Section 2 Point 7 Of The Act: 1) The Ordering Party Requires That Receptionists (Registrars) And Laboratory Workers Participating In The Execution Of The Order Are Employed Under An Employment Contract By The Contractor Or Subcontractor;
