Tenders Are Invited For Providing Care/Neighborhood Services For Seniors And People With Disabilities From The Gardeja Commune

1. The Subject Of The Order Is The Provision Of Care/Neighborhood Services For Seniors And People With Disabilities From The Gardeja Commune As Part Of The Project Titled Deinstitutionalization And Development Of Social Services In The Communes Of The Eastern Partnership Powiśle, Co-Financed By The European Social Fund Plus (Esf+), Priority 5 European Funds For A Socially Strong Pomerania (Esf+), Measure 5.17. Social And Health Services Under The European Funds For Pomerania 2021-2027 Program (Fep 2021-2027).2. The Subject Of The Order Is The Provision Of Services Consisting In Support And Assistance In Basic Living And Home Activities For The Expected 27 Project Participants, Residents Of The Gardeja Commune.3. Support Will Be Provided In The Form Of Providing Care/Neighborhood Services In The Homes Of Supported People.4. Support Will Be Provided In An Estimated Amount Of 7-8 Hours Per Month Per Person During The Duration Of The Contract.5. Support Will Be Provided Depending On The Individual Needs Of Project Participants.6. The Support Will Be Provided In A Total Estimated Amount Of 4,200 Clock Hours Over A Period Of 21 Months From The Date Of Conclusion Of The Contract. 7. The Ordering Party Reserves That The Provisions Specified In Section 6 The Number Of Hours Is An Estimate And May Change (Increase/Decrease) Because It Cannot Be Determined Precisely Due To The Specificity Of The Order, Which Is Characterized By Variability Of Needs Due To The Changing Health Condition And Life Situation Of People Requiring Help. 8. Due To The Fact That The Number Of People Covered By The Provision Of Care/Neighborhood Services Is An Estimate, As Well As The Monthly Number Of Hours Of Care/Neighborhood Services Per Person Is Also Estimated And Serves Only As An Aid In Preparing The Offer, In The Event Of A Smaller Or Larger Number Of People And Hours Of Service Provision, It Cannot Be The Basis For Any Claims From The Contractor. The Minimum Total Number Of Hours Of Service Provision Over A Period Of 21 Months Is 3,000. 9. Care/Neighborhood Services For Seniors And People With Disabilities From The Gardeja Commune Include Help And Support For The Project Participant In Basic Everyday Activities, Such As: 1) Scope Of Household Services: A) Cleaning The Apartment; B) Shopping; C) Providing One Hot Meal Or Preparing Meals; D) Assistance With Eating Meals; E) Keeping Household Equipment Used By The Project Participant Clean; F) Keeping Sanitary Equipment And Facilities Clean; G) Doing Laundry; H) Keeping Clothes Clean; I) Lighting The Stove And Bringing Fuel;J) Keeping Clean The Rooms Used By The Project Participant;2) Scope Of Care Services: A) Morning And/Or Evening Toilet; B) Changing Underwear; C) Changing Bed Linen; D) Assistance In Meeting Physiological Needs; E) Assistance In Taking Medications; 3) Dealing With Life Matters: A) Arranging Doctors Appointments, Filling Prescriptions; B) Organizing And Carrying Out Walks; C) Maintaining Individual Interests,
