Travel Agency, Tour Operator And Tourist Assistance Services - Successive Provision Of Services For Organizing Domestic And Foreign Business Trips Of Employees Of The Medical University Of Warsaw In The Field Of Air Ticket Delivery. 1. The Subject Of This Order Is The Successive (Depending On The Needs Of The Ordering Party) Provision Of The Service Of Organizing Domestic And Foreign Business Trips Of Employees Of The Medical University Of Warsaw, Including The Supply Of Airline Tickets Within 36 Months From The Date Of Conclusion Of The Contract, On The Terms Specified In The Draft Contract (Appendix No. 6 To The Swz). 2. The Subject Of The Order In Particular Includes: 1) Presentation Of No Less Than Three (3) Different Connection Variants Each Time (If Any), Taking Into Account The Most Advantageous Price And Promotion (I.E. Taking Into Account Discounts: Offers Of Low-Cost Airlines, Group Discounts, Resulting From The Deadline For Booking Weekend Connections And Other Ad Hoc Promotions), Route Length And Travel Time, In A Time Not Longer Than 4 Hours. Connection Variants Will Always Include An Optimal Travel Plan Taking Into Account Possible Rail Or Bus Connections From The Landing Place To The Destination Within One Trip; 2) Making A Reservation Within Two (2) Hours Of Receiving Information About The Selected Connection Via E-Mail; 3) Sale Of Ordered Airline Tickets, Confirmed By Sending Electronically To The Indicated Address A Ticket Issued In Electronic Form. 3. The Contractors Responsibilities Will Include Ensuring The Following Minimum Scope Of Services Provided: 1) Appointing Three Experienced Ticket Cashiers, Providing Their Mobile Phones - These People Will Be Responsible For Cooperation With The Ordering Party In Presenting Connection Options And Booking Selected Air Connections; 2) Sending Reports After The End Of Each Calendar Month On The Use Of The Contract Amount, Based On Invoices Issued By The Contractor, Divided Into: Domestic Airline Tickets, Foreign Airline Tickets, Detailing The Ticket Price Factors (Ticket Tariff, Airport Fees And Transaction Fee). 4. Pursuant To Art. 95 Section 1 Of The Act, In Accordance With The Provisions Of § 3 Of The Draft Agreement, The Ordering Party Requires That The Activities Specified In Section 2 Were Performed By The Contractors Employees Employed On The Basis Of An Employment Relationship Within The Meaning Of The Act Of June 26, 1974, Labor Code (Consolidated Text: Journal Of Laws Of 2025, Item 277, As Amended). 5. If The Contractor Designates The Persons Or Persons Referred To In Section. 4, And Such A Person Will Have The Status Of A Person Referred To In Art. 96 Section 2 Point 2 Letter E) The Act, I.E. A Disabled Person Within The Meaning Of The Act Of August 27, 1997 On Vocational And Social Rehabilitation And Employment Of Disabled Persons (Consolidated Text: Journal Of Laws Of 2025, Item 913), The Ordering Party, By Assessing And Comparing The Submitted Offers, Based On The Declaration Submitted In The Form Of The Offer Form (Appendix No. 1 To The Swz), Will Award The Offer Of Such A Contractor With A Number Of Points Of 5 Or 5, Respectively. 10 Points, Calculated In Accordance With The Principle Expressed In The Assessment Criterion
