1. The Subject Of This Order Is The Provision Of Transport Services And The Provision Of A Guardian For People With Disabilities - Participants Of The Headquarters (Ul. Morcinka 15) And The Branch (Ul. Swobodna 59) Of The Community Self-Help Home In Dąbrowa Górnicza.2. The Transport Service Will Be Provided Within The City Of Dąbrowa Górnicza, On Working Days, Up To A Maximum Of 200 Km Per Day, Taking Into Account The Participants Places Of Residence.3. A Detailed Description Of The Subject Of The Order Is Attached As Annex 8 To The Swz.
