For further details, contact on - 047-2250261 Receipt obtained upon payment of non-refundable registration fee of Rs. 1500.00 for one item and Rs. 500.00 for every additional item from the Shroff of the hospital upon payment in cash or the Bank receipt obtained upon crediting the amount in Account No. 7038649 in favour of Director, District General Hospital, Embilipitiya maintained at Embilipitiya Branch of Bank of Ceylon or a cheque drawn in favour of the Director, District General Hospital, Embilipitiya (upon realization of cheque) should be attached to the application and sent. (When sending money online or when crediting to the bank please mention the name of the institution). It is not necessary to call over at the hospital to collect applications, can be self prepared in A4 size paper as per the specimen given above. Documents pertaining to the payment should be attached to the application and the applications that are not in conformity of the specified format will be rejected without any intimation.
