The Subject Of The Order Is A Service Consisting In Confirming Death, Issuing A Death Certificate And Determining Its Causes In The Situations Referred To In Art. 11 Of The Act Of January 31, 1959 On Cemeteries And Burial Of The Dead (Consolidated Text: Journal Of Laws Of 2024, Item 576, As Amended), Regulation Of The Minister Of Health And Social Welfare Of August 3, 1961 Regarding The Declaration Of Death And Its Causes (Journal Of Laws Of 1961, No. 39, Item 202) And In Accordance With The Act Of November 28, 2014, Law On Civil Status Records (Consolidated Text: Journal Of Laws Of 2025, Item 594, As Amended) Within The Administrative Boundaries Of The City Of Gorzów Wielkopolski, Entrusted To The Contractor By An Officer Of The Relevant Police Unit, Or An Officer Of The City Guard Headquarters By Telephone. The Subject Of The Order Concerns Cases Where: 1) It Is Impossible To Determine The Primary Health Care Doctor Or The Doctor Treating The Deceased In The Last Illness, And The Death Was Not Confirmed By The Doctor Called In Case Of An Accident Or Sudden Illness; 2) The Established Primary Health Care Doctor Or The Doctor Treating The Deceased In The Last Illness Refused To Confirm The Death. Period Of Order Implementation: From The Date Of Signing The Contract. The Contract Is Concluded For A Period Of Up To 12 Months, With The Commencement Of The Service Planned For January 1, 2026. The Contract Lasts From The Date Of Commencement Of The Service Until The Date Of Using The Limit Of Confirmed Deaths, No Longer Than For A Period Of 12 Months Or Until The Funds For The Execution Of The Order Are Exhausted. The Subject Of The Order Includes: 1. The Subject Of The Order May Be Performed Only By An Entity With Documented Professional Qualifications And Authorizations Necessary To Perform It In Accordance With The Applicable Legal Provisions Or An Entity With Persons Meeting The Above-Mentioned Conditions. 2. The Scope Of The Order Includes: Issuing A Death Certificate To Deceased Persons, After Determining The Cause Of Death In The Situation Referred To In Art. 11 Of The Act Of January 31, 1959 On Cemeteries And Burial Of The Dead (Consolidated Text: Journal Of Laws Of 2024, Item 576, As Amended), Regulation Of The Minister Of Health And Social Welfare Of August 3, 1961 On The Declaration Of Death And Its Causes (Journal Of Laws Of 1961, No. 39, Item 202) And In Accordance With The Act Of November 28, 2014, Law On Civil Status Records (Consolidated Text: Journal Of Laws Of 2025, Item 594, As Amended) Within The Administrative Boundaries Of The City Of Gorzów Wielkopolski. 3. The Order Does Not Include: A) Issuing Death Certificates And Determining The Cause Of Death To Persons Who Used Health Services Within 30 Days Before The Date Of Death, Unless The Ordering Party Decides That The Circumstances Of The Event Justify Initiating The Procedure For Providing The Service; B) Cases In Which There Is A Reasonable Suspicion That The Cause Of Death Was
