1. The Subject Of The Order Is The Collection, Transport And Disposal Of Medical Waste With The Codes: 18 01 02*, 18 01 03*, 18 01 04*, 18 01 06*, 18 01 07*, 18 01 08*, 18 01 09*, 18 01 82*. 2. The Estimated Total Amount Of Medical Waste Generated Annually At The Nowodwor Medical Center Is Approximately 70,000 Kg. 3. Waste Will Be Collected Between 8:00 A.M. And 3:00 P.M. From The Following Places: • District Hospital In Nowy Dwór Mazowiecki, Ul. Miodowa 2 – 3 Times A Week; • Specialist Clinic In Nowy Dwór Mazowiecki, Ul. Paderewskiego 7 – Once A Week; • Primary Care Clinic In Nowy Dwór Mazowiecki, Ul. Mieszka I 28 – Once A Month; • Primary Care Clinic In Zakrętym, Ul. Rynek 8 – Twice A Month; • Primary Care Clinic In Cząstków Mazowiecki 118 – Once A Month; • Primary Care Clinic In Kazuń Polski, Ul. Leśna 29 – Once A Month. 4. The Contractor Also Undertakes To Collect Waste From The Above-Mentioned Places At Each Urgent Request Of The Ordering Party. 5. The Contractor Will Be Obliged To: • Keep Waste Records In Accordance With The Act Of December 14, 2012. On Waste (Consolidated Text: Journal Of Laws Of 2023, Item 1587, As Amended). • Monthly Issuance Of A Document Confirming The Neutralization Of Infectious Medical Waste Collected From Facilities Belonging To The Nowodworski Medical Center In Nowy Dwór Mazowiecki In Accordance With The Regulation Of The Minister Of The Environment Of January 13, 2014. Regarding A Document Confirming The Disposal Of Infectious Medical Waste Or Infectious Veterinary Waste (Journal Of Laws Of 2014, Item 107, As Amended). 6. The Contractor Is Obliged To Have His Own Transport. Waste Will Be Transported Using Specialized Cars By Drivers Certified To Drive Vehicles Transporting Hazardous Waste. 7. The Contractor Is Obliged To Have Its Own Or Leased Medical Waste Thermal Utilization Plant Or Have A Signed Current Contract With Another Medical Waste Thermal Utilization Plant That Also Has Appropriate Permits To Conduct The Activities Covered By The Subject Of The Order. 8.
