Request for proposals for Furniture and Office Equipment (Wooden Furniture, Chairs, Tables, File Cupboards, Almirahs, Washing Machines, Gas Cookers, Refrigerators, Television Sets etc.) Process of Applying I. Printed application form for Registration of Suppliers may be obtained either from the Head Office or from the www.aib.gov.lk website. II. Noil-refundable fee of Registration of Suppliers for each item is Rs. 2,000/-. Fees should be paid separately for every item Ser. No. (1.1 to 2.11) and submit a printed application and get registered. III. Original of the receipt obtained upon payment of the fees to the Finance Division of this institution or the Hank Slip obtained upon crediting to Account No. I (>4607 maintained at Union Place Branch of Bank of Ceylon or the cheque drawn in favour of Chairman, Agricultural and Agrarian Insurance Board should be submitted with the application. IV. All information given in the application shoidd be true and correct and the required documents (Business Registration Certificate, TIN, VAT Certificate, last years Accounts Statement etc.) should be submitted. V. Incomplete and applications without the required documents will be rejected. VI.
