Application forms obtained online only from the Website www.colombo.dist.gov.lk should be filled properly and uploaded to the website along with the bank paid slip for Rs. 500.00 made to the A/c No. 7041682 at the Bank of Ceylon, drawn in favour of District Secretary Colombo.
