Part A: Service Provided In The City Of Katowice1) The Subject Of The Order Is The Provision Of Services In The Field Of Providing Preventive Health Care For Employees, Study Candidates, Students And Doctoral Students Of The University Of Silesia In Katowice And In The Scope Of Conducting Research And Issuing Decisions On The Need To Grant Health Leave To An Academic Teacher Of The University Of Silesia - Part A: Service Provided In The City Of Katowice.2) The Scope Of The Service Includes Providing Preventive Health Care For: A) Employees To The Extent Provided For In The Labor Code And In Accordance With The Regulation Of The Minister Of Health And Social Welfare Of May 30, 1996 On Medical Examinations Of Employees, The Scope Of Preventive Health Care For Employees And Medical Certificates Issued For The Purposes Provided For In The Labor Code (Consolidated Text, Journal Of Laws Of 2025, Item 277, As Amended), B) Academic Teachers In The Scope Of Conducting Research And Issuing Decisions On The Need To Grant An Academic Teacher Leave For Health Reasons. C) Candidates For Studies Who, During Their Practical Vocational Training, Will Be Exposed To Factors That Are Harmful, Burdensome Or Hazardous To Health, In The Scope Of Assessing The Possibility Of Receiving Education, Taking Into Account The Health Condition And Threats Occurring At The Place Of Study. D) Students And Doctoral Students Who Are Exposed To Harmful Factors During Their Studies, Burdensome Or Hazardous To Health In The Scope Of: - Scope I - Assessment Of The Possibility Of Receiving Education, Taking Into Account The Health Condition And Threats Occurring At The Place Of Study. - Scope Ii - Preventive Health Care Necessary Due To The Conditions Of Receiving Education.3) A Detailed Description Of The Subject Of The Order Is Provided In Annex No. 2A And 2B To The Swz, Including: - List And Scope Of Services Along With The Necessary Diagnostic Tests Together With Expected Quantities - Annex No. 2A To The Swz, - List Of Harmful And Hazardous To Health - Annex No. 2B To Swz.4) The Contractor Will Perform The Order According To The Current Needs Of The Ordering Party, From The Date Of Conclusion Of The Contract (But Not Earlier Than December 20, 2025) For A Period Of 36 Months Or Until The Contract Value Is Exhausted If It Is Exhausted Before The Expiry Of 36 Months.
