The Subject Of The Order Is The 24-Hour Provision Of Services For The Collection, Transport And Storage Of Bodies Of Patients Who Died At The District Hospital In Oświęcim, In Particular Including: A) Collection Of Bodies Of Deceased Patients (Including The Remains Of Stillborn Children) From Places Located On The Premises Of The Ordering Party (Wards, Including Emergency Departments, Clinics, Pro Morte Rooms, Etc.), B) Transport Of Bodies Of People Who Died At The Ordering Party (Patients) To Cold Room/Dissecting Room, Located On The Premises Of The Hospital, C) Storage By The Contractor Of The Bodies Of The Persons Referred To In Point (A). A) Above, D) Handing Over To An Employee Of The Mortuary Of The District Hospital In Oświęcim A Body From A Cold Store For The Purpose Of Performing An Autopsy, If It Is Necessary To Perform An Autopsy In A Place Other Than The County Hospital In Oświęcim, Transporting Patients Bodies To And From The Place Of Autopsy, E) Preparing Patients Bodies For Release To Authorized Persons, F) Handing Over Patients Bodies To The Company Organizing The Burial With The Participation Or Consent Of Persons Authorized To Organize The Burial.2. The Bodies Of Deceased Persons/Remains Of Stillborn Children In The District Hospital In Oświęcim And In The Organizational Units Of The Zoz In Oświęcim Are Considered Bodies.3. The Contractor Is Obliged To Maintain A State Of Employment Enabling Him To Remain Constantly Ready To Perform Transport And To Ensure The Continuity Of The Services Of Collecting, Transporting And Storing The Bodies Of Patients Who Died In The District Hospital In Oświęcim.4. The Contractor Is Obliged To Always Provide Two Employees When Collecting Bodies From Hospital Departments.5. Average Monthly Number Of Bodies: 606.
