Tenders Are Invited For Providing Services In The Form Of Individual And/Or Group Corrective Gymnastics For Children And Young People With Disabilities (Residents Of Katowice)

Individual And Group Corrective Gymnastics. The Contractor Providing The Above-Mentioned Services Must Direct At Least 4 People To Carry Out The Order In Question In Order To Provide Services In The Form Of Individual And Group Corrective Gymnastics For Children And Young People With Disabilities, With Qualifications To Practice The Profession Of: Medical Doctor (Minimum 1 Person), Rehabilitation Specialist Or Physiotherapist (Minimum 3 People). It Is Required To Have A Facility/Premises Intended For The Execution Of The Contract, Located At In The City Of Katowice. The Facility/Premises Should Have An Area Of Not Less Than 150 M2 And Should Have At Least: - 1 Rehabilitation Room, - 1 Doctors Office, - 1 Toilet Adapted To The Needs Of Disabled People, - 1 Locker Room, The Facility/Premises Should Be Equipped With Equipment Adapted To Provide Services In The Form Of Individual And/Or Group Corrective Gymnastics For Children And Adolescents With Disabilities (Including A Mattress, A Gymnastic Ladder, A Bench Gymnastic Ball, Gymnastic Roller). The Facility/Premises Should Be Free Of Architectural Barriers. If Services In The Form Of Individual And/Or Group Corrective Gymnastics For Children And Young People With Disabilities Will Be Provided In The Facility/Premises On At Least Two Levels, The Facility/Premises Should Have An Elevator Adapted To The Transport Of People With Disabilities. The Premises Must Be Approved For Use By The State Sanitary Inspection And The Fire Service. The Contractor Undertakes To Ensure Fire Protection In The Premises, In Accordance With The Fire Protection Act, And In Particular To: A) Determine The Methods Of Conduct, Tasks And Responsibilities For Persons Staying In The Premises In The Event Of A Fire Or Other Local Threat, B) Equip The Premises With The Appropriate Number And Type Of Hand-Held Fire-Fighting Equipment, As Well As Ensure Its Inspection And Maintenance In Accordance With Applicable Regulations, C) Ensuring That Persons Staying In The Premises Can Evacuate Safely, In Particular By Ensuring That The Evacuation Routes Are Clear And Properly Marked. A Detailed Description Of The Subject Of The Order And The Requirements For The Implementation Of The Order In The Form Of The Provision Of Individual And/Or Group Corrective Gymnastics Services For Children And Youth With Disabilities Are Specified In The Draft Contracts Constituting Annexes No. 1A.
