Work Detail

1. The Subject Of The Order Is The Delivery, Installation And Commissioning Of The Equipment Described In Annex No. 1 (Part I) To The Swz For The Independent Public Health Care Facility, Medical Clinic. Maria Orlikowska-Płaczek In Starogard Gdański, Along With Staff Training In Proper Operation. 2. The Order Was Divided Into The Following Parts: 1) Part I - Delivery Of An Ultrasound Device, 2) Part Ii - Delivery Of A Portable Ultrasound Device. 3. Detailed Parameters Of The Equipment Are Specified In The Annexes To The Swz, Respectively: Part I - Annex No. 1 To The Swz Detailed Technical Specification Of The Ultrasound Device. 4. The Medical Equipment Will Be Delivered With The Necessary Software, An Operating Manual In Polish, A Warranty Card And A Technical Passport. 5. The Ordering Party Requires That The Offered Medical Equipment Covered By The Subject Of The Order: 1) Be Brand New, Not For Exhibition, Not For Demonstration, Free From Any Defects And Damage, Without Previous Use And Not Subject To Third Party Rights; 2) Was Manufactured No Earlier Than 2025 And Contained The Year Of Production, Type And Serial Number; 3) Met All Standards Imposed On Such Goods By Polish Law; 4) Had The Required Certificates And Markings Provided For In The Provisions Of The Act On Medical Devices, Proving The Required Authorization For Use And Marketing In Poland, I.E. The Ce Certificate And The Declaration Of Conformity. 6. The Equipment Offered On The Day Of Submitting Offers Cannot Be Intended By The Manufacturer To Be Withdrawn From Production Or Sale. 7. The Contractor Is Obliged To Deliver The Subject Of The Order To The Registered Office Of The Ordering Party. The Contractor Is Obliged To Secure Unloading And Protect Existing Elements Of Medical Equipment And Its Equipment Against Damage. 8. On The Day Of Delivery Of The Medical Equipment To The Ordering Partys Office, The Contractor Will Be Obliged To Deliver The Following Documents: 1) Instruction Manual For The Subject Of The Order - 1 Copy In Paper Version And 1 Copy In Electronic Version (Translation Of The Document Into Polish Is Required), 2) Detailed List Of Warranty Conditions, 3) List Of Authorized Service Points And Terms Of Service Provision By Service Points In The Post-Warranty Period, 4) Work And Inspection Book Quality Of The Offered Subject Of The Order (Technical Passport), 5) Warranty Card, 6) List Of Suppliers Of Spare Parts, Consumables And Consumables (If It Concerns The Subject Of The Order).
