For registration, a self prepared application as per the specimen given below (A4 size paper) should be used. A photocopy of the Business Registration Certificate relevant to the applying institution should be submitted. A registration fee of Rs. 2,000.00 should be paid for each supply and service applying for. Registration fee may be paid either by a Cheque or a Money Order mentioning the Paying Office as Ibbagamuwa Pradeshiya Sabha, Ibbagamuwa. Cheques and Money Orders should be drawn in favour of "Secretary, Pradeshiya Sabha, Ibbagamuwa" and may either be sent by registered post or hand delivered.
