Part I: Entitled: Provision Of Care Services.1. The Subject Of The Order Is The Provision Of Care Services For Clients Of The Municipal Social Welfare Center In Puszcza Mariańska, Performed In The Place Of Residence Of These People, In Accordance With Applicable Regulations, In Particular The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2025, Item 1214, As Amended) And Implementing Regulations Issued On The Basis Of The Above-Mentioned Act And In Accordance With Requirements Specified In The Swz Specified In This Chapter. Care Services Include Assistance In Meeting Everyday Life Needs, Hygienic Care, Care Recommended By A Doctor And, If Possible, Ensuring Contacts With The Environment - Art. 50 Section 3 Of The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2025, Item 1214, As Amended). Care Services Will Be Provided From Monday To Saturday, Depending On The Reported Needs Of People Using The Services, In The Puszcza Mariańska Commune.2. The Order Is Divided Into Parts. The Ordering Party Allows The Submission Of Partial Offers. The Contractor May Submit An Offer For All Parts Of The Order. The Order May Be Awarded To The Same Contractor For All Parts Of The Order. The Value Of The Order Is The Total Value Of Individual Parts Of The Order. Estimated Number Of Hours To Be Performed: 5,700 Hours For Care Services. The Estimated Number Of Hours May Change During The Execution Of The Order By No More Than 2,000 Hours (Minimum Number Of Hours 3,700, Maximum Number Of Hours 5,700). The Ordering Party Guarantees That The Minimum Number Of Hours Of Care Services To Be Provided By The Contractor Under The Contract Is 3,700 Hours. The Ordering Party Assumes That 1 Hour Of Each Type Of Service Is Equal To 1 Clock Hour, I.E. 60 Minutes. In The Event Of Ordering An Incomplete Number Of Hours, The Contractor Will Only Be Entitled To Remuneration For The Services Actually Provided And Will Not Submit Any Claims Regarding The Completion Of The Remaining Part Of The Order. 1.1.) The Scope Of Care Services Includes The Following Tasks And Activities: A) Making Basic Purchases (Food, Industrial, Hygiene Products, Cleaning Products) And Delivering Them To The Patient; B) Preparing Meals (Including Diets), Breakfast, Second Breakfast, Lunch, Afternoon Snack, Dinner - Depending On Needs; C) Serving The Client A Meal And, If Necessary, Feeding The Patient; D) Cleaning Up After The Meal, Including Washing Lunch Containers; E) Helping The Client Perform Hygiene Procedures Such As: Combing, Helping With Physiological Needs (E.G. Taking Him To The Toilet), Preparing A Bath, Etc., Helping In Entering And Leaving The Bathtub; F) Cleaning The Clients Room (Wiping Dust (Including From Window Sills)) Vacuuming, Mopping The Floor, (Except For Washing Windows), Beating Rugs, Cleaning Bathrooms, Kitchens (Washing And Keeping Sanitary Facilities Clean); G) Changing Bed Linen, Helping Patients Change Clothes And
