Sports Facilities Operation Services – 7/Zp/2025/S Access Service To Sports And Recreation Facilities For Employees Of The University Of Lodz And Authorized Members Of Their Families, As Well As People From Outside The University Of Lodz.. 4.1. The Subject Of The Order Is The Provision Of Access To Sports And Recreation Facilities For Employees Of The University Of Łódź, Their Family Members And People From Outside The University Of Łódź, Access To Swimming Pool Classes For Children Of Employees Of The University Of Łódź And Children Of People From Outside The University Of Łódź Up To 15 Years Of Age (Swimming Pool Passes) In The Form Of A Monthly Subscription. Family Members Include, Among Others: Spouses And Children Up To The Age Of 18, And Also If They Are In Education Until They Complete Their Education, But No Longer Than Until They Are 25 Years Of Age - A Detailed Description Of The Subject Of The Order Is Included In Annex No. 1 To The Swz (Later An Annex To The Contract). 4.2. The Services That Are The Subject Of The Order Will Be Provided In Two Variants. 4.3. Option I: The Services That Are The Subject Of The Order Consist In Enabling Program Participants (The Expected Number Of Passes During The Contract Period Is 3,600 People Per Month - Full Pass And 500 People Swimming Pool Pass) Unlimited Access (I.E., Giving The Opportunity To Use Various Recreational And Sports Activities In Various Sports And Recreation Facilities On The Same Day, Week Or Month And Without Declaration Of Use Of A Specific Location, As Well As Free Of Charge) Access To Sports And Recreational Facilities Throughout Poland, In Particular In Łódź And The Łódź Voivodeship, Of Various Types As Part Of A Monthly Subscription. The Order In The First Variant Is Co-Financed By The Companys Social Benefits Fund. The Funding From The Ordering Party Amounts To: - Pln 50 For The Employee (Card With A Full Range Of Services); - Pln 10 For An Accompanying Person/Child (Card With A Full Range Of Services); - Pln 15 For Children Up To 15 Years Old, Swimming Pool Card. Co-Financing From The Companys Social Benefits Fund Covers The Ordering Partys Employee And Eligible Family Members (Accompanying Persons, Child/Children With A Card With A Full Range Of Services, As Well As Child/Children Up To 15 Years Of Age With A Swimming Pool Card). 4.4. Option Ii: The Services That Are The Subject Of The Order Consist In Enabling Program Participants Registered By Employees Of The University Of Lodz Who Are Not Eligible Family Members (The Expected Number Of Passes During The Contract Period Is 950 People Per Month - Full Pass Or Swimming Pool Pass) To Have Unlimited Access To Various Recreational And Sports Activities In Various Sports And Recreation Facilities On The Same Day, Week Or Month And Without Declaration Of Use Of A Specific Location, As Well As Without Subsidies) Access To Sports And Recreational Facilities Throughout Poland, In Particular In Łódź And The Łódź Voivodeship, Of Various Types As Part Of A Monthly Subscription. The Order In The Second Variant Is Financed From The Employees Own Funds Without Funding From The Ordering Party. 4.5. The Ordering Party Reserves The Right To Make Changes In The Quantity Of The Ordered Tickets, I.E.: - The Ordering Party Will Be Entitled To Purchase
