Receipt obtained on payment of prescribed fees to the Shroff of this hospital or receipt obtained by crediting the fee to Account Number 013100129025204 maintained at Peoples Bank in favour of Director, National Hospital, Karapitiya should be submitted annexed to the application. Cheques or Money Orders will not be accepted.
