Application form for registration may be obtained from the Head Office of our Authority situated at the address given below or self prepared as per the specimen given below or downloaded from the www.mepa.gov.lk website. 02. A non-refundable fee of Rs. 3,000/ = (Three Thousand) for every item (Goods or Services) should be paid and that amount may be credited to Account No. 194109 maintained with the Borella Supra Grade Branch in favour of Marine Environment Protection Authority or paid in cash to the Shroff of the Head Office in the address given below. 03. Customers copy of the receipt obtained upon crediting the registration fee or the receipt obtained from the Authority should be definitely attached to the application. 04. Please note that payments of registration fees will not be accepted by Money Orders or Cheques. Application should be duly perfected and along with the receipt issued by the authority / customer copy obtained from the bank, photocopy of the Business Registration Certificate mid a certified copy of the customer list for the last three (03) years and may either be sent by registered post, courier service or hand delivered to the Marine Environment Protection Authority, No. 177, Nawala Road, Narahenpita, Colombo 05 to receive before 14.00 hrs on 14.11.2025.
