Tenders are invited for Supply of Other Various Consumable Goods (Boxes of Matches / Razor Blades / Needles / Thread etc.) For each item a non-refundable registration fee of Rs. 2,000.00 has to be paid to the Shroff of the Sirimavo Bandaranaike Specialist Childrens Hospital and the original receipt (Form General 172) obtained should be submitted attached to the application. In addition to that, the payment may also could be credited to Account No. 7040242 maintained in favour of Director, Sirimavo Bandaranaike Specialist Childrens Hospital, Peradeniya at the Peradeniya Branch (588) of Bank of Ceylon and a clear copy of the Bank Deposit Slip clearly mentioning the name of the institution and the Item Number should be emailed to [email protected] on the same day. Original of the Bank Deposit Slip should be submitted attached to the application. (Money Orders and Cheques will not be accepted.) Suppliers may register for one or more items and all items may be mentioned in the same application. For item No. (c) applications should be submitted on special application forms issued by this institution. Should have C1DA Grade C 9 or above registration and preference will be given to contractors registered within the Central Province. Self prepared applications as per the specimen given below along with the certified copy of the Business Registration Certificate (Should be attested either by the Grama Niladhari, / Attorney at Law or reputed Officer) and the receipt obtained upon payment of the fees may either be sent by registered post addressed to Chairman, Regional Procurement Board, Sirimavo Bandaranaike Specialist Childrens Hospital, Peradeniya or deposited in the Tender Box kept at the Office of the Accountant of this institution to receive before 10.30 a.m. on 27.11.2025. Late applications will not be accepted under any circumstance. A non refundable registration fee of Rs. 2,000.00 should be paid for each item to the Shroff Division of this hospital between 9.00 a.m. and 3.00 p.m. on working days from 20.10.2025 until 26.11.2025 and the receipt obtained should be submitted with the application.
